Whydo they over-hire?? Any managers out there to answer this? - ncMT

Posted: Jan 20, 2011

I read about this a lot, and really wonder why does a company over-hire?  Why can't they start off with a few MTs so they can feel-out the account and number of jobs?  Then, if needed, they can hire what they REALLY need.  I was hired on for a new account and they obviously over-hired for it... we're really struggling with work, or lack thereof.  I've adjusted my schedule three times too.  Even with the addition of several other accounts, I'm still not busy.  I started putting in my resume to other companies after six weeks, finally got an interview with a company and will be starting next week.  I'm so poor now that I'm going to actually work both jobs to try and catch up on the money that I haven't been making.  Hopefully I can do it for a month, that's my goal.  **Keeping fingers and toes crossed**

This is a no brainer. They overhire to meet ridiculous TAT, so there - is always someone to do the work. nm

Amen to that! - Frustrated

[ In Reply To ..]
That is exactly what happens. Meeting this ridiculous turnaround time makes the MTSOs look wonderful in the eyes of the client. In the meantime, the MTs are the ones who suffer with having to juggle numerous accounts, dealing with lower production as a result or sometimes no work at all. Very, very irritating =/

Would it be ridiculous if it were YOUR life depending - on it? Lots of people die in hospitals

[ In Reply To ..]
every year who should not have. Ideally there would be 0 TAT in getting these reports on the chart, as in front-end processing.

Ever hear of STAT dictation? Completely different than - crazy low TAT on every dictation. nm

Although some argue these figures are high, maybe - Making all patient reports stat will not work.
[ In Reply To ..]
they're worth a little consideration.

"A recent study by Healthgrades found that an average of 195,000 hospital deaths in each of the years 2000, 2001 and 2002 in the U.S. were due to potentially preventable medical errors. Researchers examined 37 million patient records and applied the mortality and economic impact models developed by Dr. Chunliu Zhan and Dr. Marlene R. Miller in a study published in the Journal of the American Medical Association (JAMA) in October 2003. The Zhan and Miller study supported the Institute of Medicine̢۪s (IOM) 1999 report conclusion, which found that medical errors caused up to 98,000 deaths annually and should be considered a national epidemic."

Virtually everyone does agree it's been getting worse since as the healthcare crisis grows. A whole lot of those deaths are from simply medication error. BTW, that's only deaths, not further damage to health, misery, etc., with the #s of those victims undoubtedly far higher.
What thread are you reading? Never said make all dictation - stat. But if doc needs a fast report, make SM
[ In Reply To ..]
THAT report stat and dr will get it ASAP. Not all reports need to be stat, not all reports need such ridiculous TAT.
You still don't get it. Doctors don't say I hope no one - kills this nice boy today; maybe I will
[ In Reply To ..]
make this this a stat. They also don't say I hope nobody kills any of my patients so I'll make them all stats. None of them actually expect any particular person to be hurt, it just happens. Like backing over a child in a parking lot. The quicker a readable report is on the chart, the safer the patient and the better care received all around.

What we do not only isn't only about us, we're the only ones who thinks it's about us to any degree at all. Most would dearly love to get rid of the regrettable need for transcription if only they could. In the meantime, there is no such thing as a ridiculously short TAT.
Man, you need to go find the thread that matches - your posts. Dont think this is the one. nm

If you choose not to follow, at least recognize one of those 98,000 - deaths annually could be someone
[ In Reply To ..]
important to you.

Providers ALWAYS needed instant TAT to take proper care of patients. Shorter TAT demands just follow technologically possible shorter TATs is all. Even if they have to go all the way to Middle Asia to get them because we don't meet this very real need.

Your pot is ridiculous. Ever hear of a phone??? - sm

[ In Reply To ..]
If it is a life and death situation, it would make more sense for the physician to pick up a phone to get the needed information or give clearance rather than dictate a report and wait for it to be put in the chart...be it 0 TAT in front-end processing or not.

By the way, believe it or not, a lot do phone and treat rather than dictate first.
HA! I meant *post*. But maybe the shoe fits. - nm

Panic hiring... - sm

[ In Reply To ..]
A company I work for thought they were getting a HUGE account and hired about 15 MTs. Well, guess what, the new account came on, but only at not even a tenth of what the expected volume was. So now we have far too many MTs and days without work, although the new MTs get work every day, while those of us who have been there a long time get nada, zero, zilch - WTH? I understand that the new hires need to be trained, but as it's been a month and the new workflow has not increased, time to downsize, IMHO. I agree with the above poster - add people as you need them - don't hire on the premise/promise of an account coming on until you know the workflow.

Vent over LOL

They can't hire as needed... - Ohio MT

[ In Reply To ..]
They get a new contract and they need MTs on it immediately because if they don't meet the TAT on the new account, they get fined HUGE penalties. It's cheaper for them to over-hire to try and keep the new account in TAT and not lose it versus losing MTs due to lack of work.

In my 20+ years of transcribing for nationals, at home, I have also found that there are MTs that commit to a schedule and then simply DONT WORK. They don't show up when they are supposed to, they don't notify anyone they are not going to work, and some don't show up at all for days at a time. In order for the company to stay in business, they have to have MTs on line 24/7 to cover the work that does exist. It's a catch-20, I know, and I hate it too, but that's the nature of the business.

I sure hope they charge a LOT for that fast TAT, - (sm) - MT Wordz

[ In Reply To ..]
because the way they're trying to keep up with what they've promised is a very expensive way to deal with it. Advertising, hiring and training a constant turnover of new employees is a pricey way to go.

A well-run business doesn't bite off more than it can chew. It grows in a controlled manner. These MTSOs trying to "have it all - right NOW" never cease to amaze me in their short-sightedness and stupidity.

Agreed....very expensive sm - Ohio MT

[ In Reply To ..]
But still not nearly as expensive as getting a big account, not getting their transcription done according to the contract, paying out HUGE (and I mean HUGE) fines, and losing this big account in the end. MTs move from company to company on a daily basis. Most don't require much training.

I agree, a "well-run business" doesn't bite off more than it can chew. When's the last time you saw a well-run NATIONAL transcription company? The big boys want in, bite off a whole mouthful, make their money and then move on. Sad, but so true.
Yup! (See my post in the thread below this one) - MT Wordz

May 17, 2014

If you monitor this board, then you surely have a modicum of an idea of how unhappy we MTs are with your so-called management.  "Exciting News" doesn't cut it.  FIESA (a.k.a. FIASCO) is only something to demoralize and demean us.  The Link is a waste of time.  MT Week and its games, etc., is meaningless.  When are you going to do the right thing and take note of what we've been saying to you all along?  When people just don't care any more, when they ...

If any TSMs, Regional Managers or executives are reading posts on this site, I don't know how you sleep at night!  We, as MLS', know exactly what is going on, even though you think we are stupid.  The only reason you have implemented the "Patient Safety Compensation Plan" is to pay us less so Nuance can make more money, it has nothing to do with patient safety, it is corporate greed so STOP all the lies!!! Also, please save all the crap at the end of the "updates" ...

SoftScript - How It Is To Work With Managers, Etc.
Jan 06, 2011

Nuance Only Pays Their Middle Managers Well
Jun 13, 2014

I personally know someone who works as a middle manager for Nuance, makes well over 50,000 and has had constant raises since being hired.  From what she tells me she is very effective, but even the other middle managers who are not at all effective with huge make up costs and teams who fail audits constantly do not get terminated.  Plus, they do not have to be MTs.  They just sit in their home office and write rah rah emails to their team to keep them going and make sure they cove ...
