Working for one of my prn jobs yesterday and had to look in the patient's chart to get information. Found a hospital discharge summary on the patient dictated by a doctor I also type for in a different group and was appalled at the poor quality of the transcription. In the first line was transcribed that the patient was transferred from HAMSTER Hospital - nope, no Hamster hospitals around here - then an easy blank in the review of systems and another in the physical exam plus left out part of the name of the group I happened to be working for yesterday that anyone familiar with the area would have caught. These were just mistakes I caught when skimming the report, who knows what I would have found if I had read the whole thing. I don't know which company does their transcription, I do know it is an India company and the ladies who worked at this hospital for years had to apply to them to keep their jobs. Toward the end of the report the correct name of the hospital the patient was transferred from was typed twice, so where was the QA who should have caught it if the transcriptionist didn't? This is a plain English speaking doctor and everyone I work with loves typing his dictations because he is so easy, just makes me crazy to see such lazy errors!
