Help Poor Sound Quality Report - pulling hair out

Please HELP!!!  What is the policy when you have a 22 minute dictation and the sound quality is so poor you can only understand may be every 4th or 5th word/phrase.  I already have about 25 blanks and I'm only about 5 minutes in.  I have been a tier 3 MT for 12+ years and am not trying to pass the buck here.  I believe it is actually a mechanical malfunction on his end.  There are times when it is dead silent and I can't tell if he is dictating or not through it, when I can hear again it doesn't make sense.  Please help!!!

I listen to the whole report, inserting blanks as necessary, then send to QA with a note saying "poor sound quality throughout entire report" or whatever. I feel for ya. My morning has been gum-chewing, mint-clacking, nurse-flirting, smug cardiologists.

It would depend on the CP. Also, if I don't think there's a snowball's chance in Hades that the QA will be able to understand anything either, I will bypass QA and send directly to client with the same note.

to QA with a note that the sound is so bad you can't transcribe it. Then send a note to your CCM with the job number and a description of the problem and that the report is untranscribable, and check the email high priority box.

But first, scan the recording to make sure there aren't spots that the sound problem clears up. If there are decent spots, you must transcribe them.

I would type what I can, leave blanks and send to client with note stating very poor audio. I would not send to QA, as I do not send anything to them because I am not getting penalized for things I can't do anything about. I have even suggested in notes that they may want to redictate the report. Just keep your notes polite and send right to client. I have never had a problem doing this and if I do, tough. If I can't hear it I can't type it and won't be penalized for it.

The report turned out to be 133 lines with 85 blanks. I sent it along to QA because I felt wrong sending it to the customer in the condition it was, even with a note of explanation. I also informed my CCM and QA sup regarding this and was informed, that while I could have sent it to the customer (QA couldn't fill in the blanks either) since I did send it along to QA it will count against my 5%. Makes one really think twice about sending anything to QA. Not really in the best interest of the patient, just my opinion.

I go through it as quickly as I can, leave all kinds of blanks, mark it for QA and let them deal with it. I spent NO extra time on these report. Some of the blanks I could probably get, but I figure I'm going to get the most out of the money I'm losing sending to QA.

Why would you feel bad. Did you cause the audio to be bad? Why would you send to QA and risk getting penalized? I don't send anything to QA. Are you going to be on here in another post complaining that your pay is cut because of QA submission? You need to get over taking responsibility or "feeling bad" because of something that is not your fault.

my time (or the company's, for that matter) on it. The reason I get bad sound on my account is that they won't upgrade their dictation equipment, and it's ancient.

I let my supervisor know the report is untranscribeable, and I send it back. Will I get dinged? Yes. But not nearly as much as I would get dinged if I wasted hours trying to transcribe it, left a ton of blanks, and sent it to QA, as QA dings for EACH blank.

I can then make up for whatever amount I am dinged by returning the bad report, by typing lots of other reports that ARE transcribeable.

To waste time on the impossible-sound-quality reports hurts the company, too. Better for them that you type 10 good reports that they can bill for, than waste time with a dud that they'll ultimately have to ask to have re-dictated.

If the offending account with the ancient dictation system has to re-dictate enough reports, maybe they'll eventually figure out the fact that they need to invest in some up-to-date equipment.

I know my hearing has not changed but it seems that the sound quality of the dictation lately has changed.  It doesn't matter whether I'm using a cphone or downloaded voice files.  On A LOT of the dictations there seems to be a whooshing sound (for lack of a better description) in the background.   On others there seems to be a squeaking sound as if the doctor is playing with the phone cord.  Is this because the docs are now dictating directly into the computer ...

Almost ALL reports done so far this a.m. have had the worst quality.  Dictation cutting in and out, muffled speech, turning head away from phone, shuffling of papers, talking in background.  COME ON!!!  This job is difficult enough.  Quit wasting my time on this garbage.  ARGH. ...

Trying to finish my shift for Mmodal.  Doing maintenance on phones, but what a ride it's been trying to do transcription.  The audio is all garbled like an old loop system that has been used to the max.  Leaving more blanks that transcribed product on the page.  There are going to be some  unhappy docs over this. ...

sooooo...I just received an email from the quality person of the day.  She said the client requested an audit of all reports for a certain provider based upon poor quality.  Two of my reports came up on this audit as being below 99.5.  These reports had 1-point errors on them because of omitted and/or inserted medical words.    I am not denying that I made the mistakes.  However, if there is a consistent quality problem with a certain provider, perhaps...and I kno ...

I'm trying to troubleshoot something.  Does anyone else have sound-quality issues on DSG dictations?  Just curious if I should seriously have this looked at on my end.  It's not all reports, but enough so that I need to look into it one way or the other.  If I'm the only one, I'll hire someone to look into this.  I have no problems with audio otherwise, just work-wise. ...

I am having a very hard time with sound issues.  I turn it all the way up and still struggle just to hear the voices.  It doesn't matter if I plug my headphones directly into the computer port or the speaker port.  It plays louder if I let the speakers play out loud without headphones but I cannot always do that when others are around because of HIPAA and privacy issues.  I have a generic SoundMax sound card on a Dell computer.  Does anyone have any suggestions to m ...

Why would one by one doctors who I have listened to for years suddenly have changes in the quality of their voice files?  They were clear as a bell and now muffled or with static.  What could be causing this?  I'm afraid the company I am working for is planning on sending files overseas but I can't figure out why the sound quality would change.  Does anyone have any ideas? ...

I have a service that rerecords from the hospital server and then we download from the service's server.  The sound quality is absolutely atrocious by the time it comes to the MT with most jobs sounding like they were dictated on a cell phone inside a cardboard box.  I am presently using a Boostaroo which make the audio louder but does nothing to clean it up.  Is there something I can buy that will improve the quality?  TIA ...

I have done absolutely everything that I can think of and it's still horrible!  I upgraded from WinXP and Word 2002 and bought a brand new, faster computer with Win7 and Word 2010.  I bought the recommended USB headphones.  Although Spectra headphones aren't considered very good quality, the USB made the sound louder but not clearer.  It still sounds muffled.  I noticed absolutely no difference in the sound quality when I went from the old computer and the USB ...

Has anyone ever complained through Remedy about the sound quality for Fluency?  I realize this is probably futile with MM, but it has become so unacceptable. Current Probity employee about to be sucked up by the impersonal big company.  Anyway, has anybody noticed in the last two weeks all the good dictators are gone and there's nothing left but the garbage dictators and poor sound quality?  I have struggled all evening and have a total of about 300 lines.  Ran out of work both Saturday and Sunday nights which has never happened in the past.  Either I'm on the s**t list or the work is already transitioning somewhere else ...

I quit in early 2014 because there was a sudden switchover of doctors in the acct I was typing, and they were the worst I've ever encountered. The sound quality was atrocious at Nuance. It was like scraping the bottom of the barrel. I work for a different company now that doesn't make you jump through stupid FIESA hoops, etc, and it's nice. I ran an error report for all reports, all accounts on my platform for this quarter, 10/1 - 10/18. So far the Quality Index is 84% vs Quality Score 99.18%.  Didn't they advertise this new error scoring as working out in our favor?  Doesn't look like it to me. This is based on 2194 jobs and 105,358 lines audited on E5.  I checked the box that says view all corrections on the correction report. ...
