Extremely poor treatment - anon

I work for 96 physicians who (rightfylly so) expect the best and fastest quality transciption. However, when seen as a patient, I get the most horrid treatment (and fradulent) treatment possible (most most by clients.) I not only receive "steroid" injections that obviously have no steroids (only receive a few hours of pain relief) to dental care with crowns falling off after 2 weeks (I have had temporary crowns on as long as 3-4 years). These temporary crowns are put in and appointments rescheduled for several months to a couple of years.................. still waiting for root canals. I know if I had given these physicians or dentists the quality of work they had given me, I would be fired on the spot. What gives?

Why are you still going to these providers?

Also, it is not uncommon for steroid injections to not work or provide little relief. It depends on the extent of your issue. It doesn't automatically mean there are no steroids in the injection.

before I hire anyone to work on my house, my car, my pets and me. Not perfect, but better than going to some of these places blind. Also you can write reviews of your experiences.

Remember the good ole days when you could hear the doc outside the exam room door grabbing your chart and reading it before entering the room? Now, just a grab and a quick sit down in front of the computer monitor while they look confused and press buttons and try to operate the mouse. This goes on for about 5 minute with comments like:
Doc: "Oh, you lost 25 pounds since your last visit"
Me: "Uh, no I'm the same - that was an erroneous entry"
Doc: "So how are you doing on the Methadone?"
Me: I don't take Methadone,and have never taken meth, heroine, amphetamines, etc.
Doc: "Oh, they forgot to get a urine sample"
Me: "I'm here for conjunctivitis!"
Doc: "Oh" (another 5 minutes at the computer trying to figure out how to make corrections.

This is what EMR has done to the quality of care, not to mention all the demographic errors because they now leave entry of this data up to just anyone including the doctors!

He comes into the exam room to supposedly see me for a shoulder injury but instead goes on for several minutes about my thyroid tests and treatment. Nothing is wrong with my thyroid, which I tell him. He then looks puzzled at looks at the chart, stating, "Sorry, wrong patient. Well, we are out of time. Goodbye." He then charges my insurance company several hundred dollars for an exam.

On the steroid post, I have been having these injections for 8 years and definitely am aware of their effects, both when receiving the injections and the duration of relief I am supposed to have. There were none in my last injection, and very little in others. I then read a referral paper for shoulder injection he could not perform, and sure enough he listed me to receive only 20 mg of Kenalog, when I am supposed to get 40 mg. I specialize in pain management and type doses all day long.

You just described my typical doctor-visit! - If I could see a veterinarian, instead, I would.

Now these are just from the past say 4 months. ER visit: Was listed as being on HIV medications, news to me and went back to get the head of the ER department to remove those. Saw my weight recorded and from 1 week to the next had gained 100 lbs- OMG, I sure hope not but there. The latest one was from a cardiologist's records and it showed I had a leg brace. Again, not so. I cannot believe all the errors you can find in your own record.

I have been transcribing for 11 years and everytime I get a fasting talking dictator with an accent, it slows me down so much that it affects my production. I have slowed down the speed and added several expanders but that does not seem to help. Are there any tips or suggestions anyone has that can help? Thanks, Mel ...

I have been a transcriptionist for 15 years, all of which has been on Dictaphone EXText and now EXEditor. Why does my ESP in Dictaphone keep interfering? If I use an expansion and hit spacebar the ESP windows comes up, which makes no sense because ESP uses ctrl-ctrl to open? And how on earth do you learn it all? How will it benefit me after using thousands of ESPs daily. Help! ...

There are some weekends where the dictation is tolerable. This must be MM's "idiot dictators weekend" because that is all I've had today. Ranging from the moron who tries to dictate with so much static on the line that I have to leave 15+ blanks because SR didn't "recognize" anything to the dumb one that dictates 300 seconds of a report but leaves the phone off the hook so I get to hear conversations, intercoms, beeping, phones ringing, banging, etc., for 1500 seconds. ...

Is it me or is it that the sound quality is getting worse and worse on these accounts. Ever since last year when they had the town hall meeting saying they were making improvements (e mails, etc.) and then they got bought, seems all that improving stopped. No more microscopic QA dissection for me. My new plan is to shoot the job full of blanks and send it in - far less stressful for sure. Each and every time I hear something that I am not 100% sure of - they get a blank. This is not now nor was it ever MY personal problem. I know my hearing has not changed but it seems that the sound quality of the dictation lately has changed. It doesn't matter whether I'm using a cphone or downloaded voice files. On A LOT of the dictations there seems to be a whooshing sound (for lack of a better description) in the background. Please HELP!!! What is the policy when you have a 22 minute dictation and the sound quality is so poor you can only understand may be every 4th or 5th word/phrase. I already have about 25 blanks and I'm only about 5 minutes in. I have been a tier 3 MT for 12+ years and am not trying to pass the buck here.
