Do doctors get "graded" on dictations? - Cheesey

Seriously, if not, they should.

 

Some docs just repeatedly have horrible dictations. I'm not talking about accents or anything like that. I'm talking such horrible mumbling it's ridiculous. Or talking to far away from the mic. Or apparently trying to dictate in a hurricane. Or trying to dictate over a crying baby. And on and on.

Once in a while, fine. But when it is constant...when I see that doc's name get pulled up and I instantly know the horror that will soon follow...seriously, they need to know how bad their dications are.

Constructive criticism. Like "hey, could you perhaps enunciate more? Maybe help the MT out on whether you're saying 15 or 50 since those kind of run together?"

or "Next time, could you perhaps get a little closer to the mic. Sometimes it's hard to hear your voice."

Nothing mean, but seriously, when people point out things I do wrong, I learn GREATLY from that, and rarely do it again. I become more conscious of it. As an MT, I'm given this criticism all the time, and I love it. This feedback only makes me better. And if docs got a little note on how to improve their dictating ability, why, the world just may be a happier place.Laughing

That is, if they learn to take it with a grain of salt. I'm not trying to tell them how to dictate, but meeting somewhere in the middle will just help us all.

I have heard from people who worked in house for many years (me not being one of them) that in the "old days" they could do that. The transcription department and manager had a good relationship with the docs and if the doc gave them crappy dictation, then it was addressed with him. They knew that bad dictation equaled a bad record and cared about it.

Unfortunately, with the advent of the MTSO, those days went away with the 13 cpl or more that MTs used to make. The competition for clients is so brutal right now that no MTSO is going to risk rocking the boat because that facility or doctor's office just might find a company to do their work for the same price or a half cent lower who won't give them "a hard time" about the quality of their dictation.

But it's run like an MTSO and I'm pretty sure no one is saying anything to those doctors. We're just expected to put up with "challenging" dictators, not to mention the horrible audio. I think "challenging dictation" used to mean searching for the obscure term or new equipment word or whatever. Now it means trying to figure out what some whispering speed mumbler is trying to say.

It used to be like that when we worked in-house and had a relationship with the doctors. I never had a problem with them being insulted or offended if we mentioned something. Just another thing these large MTSOs have changed for the worse. They just deal with the hospital administrators and along with that the inherent lack of communication with those who are actually doing the dictation.

It actually isn't just MTSOs that seem to have a difficult time honestly communicating in a constructive manner without fear these days.

At one time, I worked in a hospital where the HIM director would never confront a dictator. Either she was a coward or just didn't care to take the time to honestly confront the offender. Of course, she wasn't the one actually doing the transcription/editing.

IMHO, confronting someone and giving them an opportunity to improve is actually very kind, and allowing them to continue in their apparent ignorance is actually very cruel to both the dictator and the transcriptionist/editor.

A world where honestly among all would help us flourish without a negative result...this is no longer anything but a dream.

We now live in a world of cowards or people who do as little as they can get by with when it comes to interacting with the source of these transcriptions, or so it seems to me.

On a similar note, I recently became old enough to be eligible for a discount at a local restaurant. I looked forward to going there for dinner and getting my "senior" discount. At the checkout, I went to show my ID and was told that they are not allowed to ask for ID, but they would take my word for it. As it turns out, I could have gone there months ago and just lied, said that I was already of age and received the discount.

What a sad world where one providing a service does not feel comfortable with direct communication and chooses to let the customer get away with being less than honest as was the case at the restaurant, or continue with bad habits as is the case with medical transcription.

25 Second Dictations

For the past 2 days, I have been on an account with very short dictations - clinic notes that average between 7 and 30 lines.  I am making less than minimum wage - it takes a certain amount of time just to do the ADT info and of course they are all ESL doctors.  I don't have a primary and I certainly hope this account doesn't end up as my primary.  Anyone have any tips on how to make money on these short little dictations? ...

Seriously, I wonder if there is a way to record these dictations. I would LOVE to hear for myself the "errors" QA/QC are always finding and putting into Fiesa. The fact that we are not provided with proof of these "errors" really concerns me. Please Nuance bigwigs, prove to me that QA/QC isn't making things up or hearing things wrong. Really, are we to believe that we are the only ones making mistakes, that they always have perfect hearing? I want proof! ...


Partial Dictations

I think all dictations at Nuance should be partial dictation worktype. Wtih "partial dictations, you don't have to do as much checking of demographics, figuring out date of service and filling that in, etc., and not having to pend and leave comments so mcuh. I notice I'm about 75 lines faster an hour when it's partial dictations because there's less outside-the-report activity to be done. This is extra activity we dont' even get paid for. Mistakes In Normal Dictations

At my company (a smaller one) there are a lot of mistakes in the normal dictations that the doctors request.  The way they are created in the first place is the transcriptionist types up the report and sends it to QA saying the doctor would like to use this in the future for his normal, but I don't think they get reviewed by QA.  I think they just get created the way they are.  So if it was a right eye, the word right is there all the way through it.  If a word is misspe ...

Worked as an IC 20+ years. Now having to work for a service. The account I do rarely has a dictation longer than one minute. It takes It takes a LOT longer to research, log demographics than it does to actually transcribe. At home I did 1200 to 1800 lines a day. With this account I can't even do one third of that! The doctors apparently have no idea that their MTs are in different parts of the US, that we have to research the spelling of doctor's names, counties, cities, faciliti ...

The WHOLE report didn't have three words consecutively that made the least bit of sense or had anything to do with what was dictated. Doesn't it drive you nuts when a doctor consistently cuts himself off when he dictates?  I sometimes get half words, the last couple letters of words, places where I haves to leave blanks because they say something like:  EKG showed (silence) rhythm.  What kind of regular?  Regular rate and....?  Arrrrggggghhhh! encounters, are they required to have a short course on proper dictation procedure? I would think, being that this medical record is sooo freaking important that we are drawn and quartered if not perfect, that these professionals who generate these reports would also be schooled about proper pronunciation, enunciation, etc. before they throw it out there for us to perfect. Getting kind of sick of these lazy, mush-mouthed, slurred, rapid and mispronounced dictations. ...
