Even though we are paid by production, a company could say they don't pay for the words "the" or "patient"? Because IMO not paying for headings is equally as stupid and ripping us off as much as that would be.
I literally cannot wrap my brain around how that is legal.
Though, I heard about a company a long time ago that didn't pay for spaces too. I don't know if they are still around or not but that space bar adds up to a lot of pennies too!
