I have to agree. I, too, have pulled up previous reports to check on meds or allergies and have been horrified by the quality of the work. Just sloppy mistakes and, in one case, the patient changing gender throughout the report! Yes, undoubtedly that's what the dictator said (shame on him, and don't you wish we got bonuses for correcting their "critical errors"? But that's food for another post), but just a little common sense and a quick proofread can really move you up that dreaded grid. It is designed to pay more for higher quality, people.
