Except for the temperature, none of the values were as dictated. Dictator states there IS a LITTLE, output is there WAS a LOT. I have been receiving more and more made up reports - as in this is totally not what was dictated, the dictation was very clear, and yes, this COULD be what goes along with the patient's problem, BUT IT WAS NOT! Too much auto output. Meanwhile..... doctor dictates "the patient was vomiting," but....... no.... what...... MM IT knows it really is 'the patient has no nausea or vomiting.'
