I take breaks whenever I need, usually short ones 1-3 minutes to get up stretch, walk around.
When I find my mind wandering, usually because of boring repetitive reports, I will wake it up by looking up something I want to know. That takes 1-2 minutes.
I do not belittle myself for taking breaks because I've found if I try to push through when I physically tired, I just end up making mistakes. If my brain keeps wandering I need to take a quick brain break to wake it up again.
What has helped me the most is getting set. I dress for work and get in the mood so that nothing distracts me (except my own mind).
Another thing that really helps me is to try to "beat" ASR. I know it often puts in wrong words, so I make it a game to try to get every wrong word (perineum instead of perianal, peritoneum instead of perineum). This keeps me really focused.
I waste no time. I do not sit there waiting for the dictator to say something. I read ahead and fix obvious errors while the dictator is saying nothing at all. So, in essence, the audio is playing or I am keyboarding or doing both almost all the time.
I usually put a timestamp beside anything I am not 1000% sure on. I relisten after I've gone through the entire document once at a higher speed, then slow down.
I go through the first time at a fast pace 100% to 110% of speed, and then go back to the timestamps I left at a slower speed (even as slow as 50%). I find that the speed up, slow down, speed up, slow down, really eats a lot of time. So, fast speed all the way to the end, do not worry about a word I cannot get, put in a timestamp. Maybe the dictator will say it more clearly later.
Then I go back through all timestamp blanks at a slower speed.
I tend to google a lot of blanks at once, speeds up the google issue.
