We query the physician if their documentation doesn't make sense or is conflicting. If it is a transcription error as in, hyper instead of hypo, which shouldn't happen but it still does, we take it to transcription manager and she fixes it and keeps track of those errors. There have been some that are a wrong word which resulted in a whole different diagnosis. We can listen to the dictation too to check what they really said. We are very tuned in to the encounter, how the diagnoses are forming as they go along, and these things really stand out.
