Some hyphen rules state that you don't use them if the word ends in "LY," but I've read that that only applies if the "LY" word is an adverb, like a "quickly receding hairline" or "slowly increasing lab values," so there may be some difference of opinion on that.
If the doctor had dictated it like: "This is a middle- to elderly-aged Caucasian woman....," then that's how I would type it.

Does ASR not "hear" year-old approximately 5 million times a day? This cannot be because someone "changed it wrong" in an ASR report. It is because it is stupid and whoever programs it is stupid.
Also it takes 6.1 about 3 seconds to change the speed in a report now - up or down. I have to sit and wait.

I personally know someone who works as a middle manager for Nuance, makes well over 50,000 and has had constant raises since being hired.
