Stroke/neuro disease specialist speaks with lisp and is eating while dictating to boot (always fun... LOL!). He is giving Past Medical History on patient, so his specialty actually may not be relevant. She is being seen for Alzheimer's with visual hallucinations. Other dictated PMH includes hypertension, diabetes mellitus type II. (Obviously not complete, given her current meds are Synthroid, atropine, vitamin E, Aricept, Lipitor, nifedipine, Namenda, glyburide. (Giving you all I have in hopes something rings a bell with someone out there.)
The entire sentence is: "She has a history of [s/l ?**e-log-a**?]."
