What do you do when a doctor makes no sense? - dnr

Leave one huge blank, or leave it exactly as he dictated? I have no idea where the doctor is going with this sentence.
I would leave a blank at - sm

the beginning of the sentence, but leave the sentence the way he dictated. Send it to QA with a note. I sometimes send the job number with a short note about the doctor to the PC in case the client questions it.
I type it as best I can exactly - sm
