I really think they insert words while auditing to make us look like we're making mistakes when we aren't.  It makes them look like they're doing their job, and keeps us on edge, which is what they seem to want.  That's not to say we don't make occasional small errors, but so what - they should be kissing our feet for the good work we do provide.  Unless we copy every single report we do to compare with their interpretation, we have no recourse to dispute that the offending word was there or not in the original report.

Some of the words they have listed on my audits, I wonder how they got there!

I did edit a report with the name of this young girl patient and it was spelled a little unusually. Well, I'm SURE that I replaced every one of those names with the correct spelling of her actual name. They have it listed on one of the reports that I missed it when I'm sure I corrected all of them!

I think keeping a copy is against HIPAA rules, but I believe they know what you typed and what QA types.

I'll be editing along and maybe hit the up arrow key and a '6' just appears. I'll stop and think if there was any way I could have hit a 6 key. Very strange. And the headings it inserts. If it doesn't match what the dictator says and I ctrl+shift+H to change it, then what the dictator says comes up. Sigh. Odd system.

I thought I was going insane. Things pop up that I KNOW I didn't put in there. Like patient names, if they are weird, I copy them from the ADT and use that, yet got gigged for spelling a name wrong, I doubt it...And yes keeping a copy is against HIPAA but who cares, they gonna take my birthday?

If anyone ever found out about violations to HIPAA, you could be prosecuted and fired, fined, or jailed. Also, I would not advertise that you are willingly violating HIPAA as it would be the obligation of any good employee to report that that kind of activity is going on.

I think the judicial system has bigger fish to fry. Besides, any company that sends 99% of its work over to India - where there ARE no HIPAA regulations - probably doesn't care much about patient privacy anyway.

Please Google HIPAA. It covers a lot more than just copy/past/save. As a business entity, M*Modal signs a contract with their clients and must abide by all the same HIPAA rules that a healthcare facility does. This responsibility also trickles down to all of M*Modal employees no matter what country they are in.

REGARDLESS of all the "legal mumbo jumbo" that MModal signs, agrees to, yada yada yada ... the *REALITY IS* that any company that allows its work from India to be sent directly to the client and placed in a patient chart with the name/identity/CC copy recipient errors that I see in 99% of my day, OBVIOUSLY does not care one bit about HIPAA violations or patient security.

They only come across my desk because there are blanks to fill in and the mistakes sometimes make me GASP out loud. It makes me wonder about all the craziness being sent directly through to the client without it ever being caught. I understand about HIPAA and all its *intended* importance but the truth is that there are probably THOUSANDS of HIPAA violations being committed by our foreign employees every single day. Trust me ... a domestic employee copying and pasting a few reports onto their computer is a teeny tiny little fish in the ocean of violations being committed every single day by the ILPs.
If saving a redacted report is against HIPPA rules, then the repository is a violation. - nm

Keeping a copy for reference purposes is not a HIPAA violation.

HIPAA prohibits disclosure of PHI/PII ... DISCLOSURE ... to unauthorized third parties.

The facility can disclose to the MTSO and to you (business associates). You can keep copies, but you cannot disclose them to unauthorized persons. It is unwise to keep copies because they can be disclosed by accident when your computer requires repairs, is discarded, or is stolen, when they get emailed, or when printed material is discarded.

HIPAA violations are now being taken to court. Fines are imposed on a per-violation basis. One group just got a fine in the range of 8.5 million.

Just FYI, misspelling names and typing the wrong names is NOT a HIPAA violation. Typing incorrect drugs and dosages, operations, or genders is not a HIPAA violation. HIPAA has nothing to do with the CONTENT of medical reports. It only deals with inappropriate DISCLOSURES.

The burden of this rule falls upon the entity that DISCLOSED the information. If your hard drive containing stored reports or your trash with identifiable information in it is stolen or discarded, you are responsible. If your employer's data containing PHI is stolen, they are responsible. If a volunteer or employee steals PHI from unsecured computer screens or printed paper in the facility, the facility is responsible, along with employees who left the material unsecured.

If reports are faxed or emailed to unauthorized parties, the person and facility who/which faxed or emailed is responsible. YOU are not responsible because you didn't DO the faxing or emailing. You cannot be liable because you entered the wrong Dr. Smith, because you have no way be sure who it is and the faxer/emailer should know better than to fax PHI/PII.

Faxing and emailing is so dangerous that facilities are stupid to allow it. Many no longer do, but doctors continue to dictate cc's.

You can "redact" reports all you want, but removing names and numbers isn't enough. Any information that can be used to determine a patient's identity, alone or in combination with other information, is PHI. The surgeon, the procedure, and the date or an unusual feature is sometimes enough. It is difficult to redact reliably.

The problem with copies is the risk of disclosure for which you would be liable. There is also the possibility that you could be considered to have stolen the PHI, leading to criminal charges.

Can somebody tell me what this audit/QA procedure is now?  I'm so confused with when they are done, how many reports are done, etc., etc.  I had been told earlier that 5 reports would be randomly pulled monthly and checked.  You would be notified you had been audited and the results.  If you passed, another 5 would be done the next month and so on.  If you didn't pass the audit, then a redo audit within 7-14 days would be done on another 5 random reports.  ...

I don't understand how auditing works.  Two of my reports were reviewed, and in each case, a word I had puit in had a line through it, but then it was replaced with the same exact word!  For that, I got marked off, getting only 99.50% or 99.75%.  I wish there was a way we could give direct feedback to the auditors at the time we're viewing our reports. ...

:(   I have tried to give the QA program the benefit of the doubt, thinking it was being done fairly, rewarding us for near perfect QA scores.  I have managed to keep my QA score above 99% every week until last week.  Before last week, I normally have 8 to 11 reports pulled for audit, some short, some long.  Last week I had 1 critical, 1 major, and 2 minor mistakes which dropped me down to no bonus pay.  Not faulting anyone for that, totally my mistakes, legitimate errors.  But....once th ...

I have only had 3 audits in 2-1/2 years. Even though I received notice of scores of 99.6 or 99.7, I did not receive any account of what my mistakes were to produce those scores. A comma? A hyphen? QC Audits I Never Knew About

Okay, I may be the last one to know this, but I thought the only QC audits I was getting were the ones showing up on the DQM QA Feedback screen where it says (1 unread) or (how many ever unread). When you click on them, if the report has been audited by your QC, it will have a percentage score.  But that's just for the reports you sent in with a blank. This is silly Not only do we need to follow CP guidelines but BOS and then DocQscribe standards. So even if you have the first 2, the third will get you audit.    Why can't they spend the money and create one set of rules to follow and be done with it.  We work on production now.  checking on previous dictated reports and then double checking guidelines from all three areas ??  and when you send in a blank now I am also being questioned about the audio etc.&nb ...

I got CRITICAL errors on audit for things like:  I typed "I used a translator telephone..." (that is what I heard) and the person auditing changed "a translator telephone" to "the translator telephone" (I suppose she heard "the" instead of "a"). In the physical exam portion of one report, the dictator used "was" throughout except for one time he said "is" and so I changed it to "was" to make it consistent and got a critical error for that. Please tell me how these are critica ...

Does it seem like no matter what you do it is not good enough? Here lately it seems like they only audit the worst dictator. The only reports they have audited for me lately have been ESLs lately. Maybe that is their way of trying to get rid of me. Maybe they want to thin the herd and just send it all to India. I just hope to find something soon outside of MT. I have been looking.   ...

I'd like to say I have had to learn accounts on my own and have had to cross reference many, many times.  Yes, I have seen blanks where if someone had taken the time to hunt for the word, it would have helped me also, but for the most part with what we have to work with, I have seen words that are highly inaudible by many, many doctors with bad dictation and yet what was typed was a wonder we could make anything out of it.  I have seen no help from the hospitals themselves as ...

Just wondering if anyone else has had this happen lately.  I received a quality audit on 4/25 on my audit, which I passed at 98.3% (had one icky one in there).  Then on 4/29 got another email of an audit of 5 reports, did not give a total score, but they were all pretty darn good (3 at 100%, 1 at 98, 1 at 98.75).  Then on 4/30, another email, they checked 3 more reports (all those above 98% too).  But what is going on, are they searching for something bad?  Are they doin ...

I am NOT going to read all those posts, but I just wanted to let you know that you are lucky you only get audited once a month.  I get audited DAILY, at least once a day.  Today I got audited twice, one 100% the other 99.75%.  If I can remember correctly, I got dinged for typing something like "Aorta was tortuous" instead of "The aorta was tortuous".  It was for an account that has a multitude of dictators - I swear there is a new one every day and no two on a consistent basi ...

I work on a very difficult account as I'm sure many of you do as well, and I spend a great deal of time listening and re-listening to these difficult dictators to make sure I am hearing correctly.  I just checked my audits again and see that I have received another 1 point deduction on a post audit for something they heard differently that I apparently did.  It frustrates me no end that ...
