just wondering... audits - so frustrated

Just wondering if a transcriptionist with too many years of experience to tell fails an audit, then how are these India MTs keeping their jobs.  Are they passing their audits?  I just have a hard time believing that they are.

This company used to be good to work for. I am wondering if anyone has heard of being fired for giving notice. I am seriously considering this, but may want to chose my time wisely.

Just got dinged on PT/INR - isn't this correct? Told should be "PT INR". If that is the case, then there should be an "and" in between - right? Also got dinged on "dorsal supine". Told should be "dorsosupine" - but have NEVER seen this in print. Anybody agree or disagree?

PT and INR are 2 separate lab studies. They are often done in conjunction, hence a forward slash as in your example. Maybe a comma in between would also be acceptable.

Dorsosupine sounds like something invented by the dictating physician (or whoever did your audit). I would have transcribed it as you did, which would be the correct interpretation.

Unfortunately, it seems the CORRECT interpretation is not always correct.

dorsosupine is not a word. It is dorsal supine. I would fight it out with your QATL.

As the other poster said, PT and INR are 2 separate labs and therefore need something between them, PT, INR or PT/INR or PT and INR.

You need to report this to the QATL and ask for documentation. I have reported many errors and was given a thank you for reporting these errors.

I've been engaged in an all out battle with "dings" like this. It seems to be a preference and is totally contingent on the person doing the auditing! My CCM has been amazing and is constantly running interference for me. It has totally got me second guessing what I hear now though! I have the volume CRANKED UP on things like what you mention to make absolutely sure, yet sure as poop, they come back.

I am keeping track of these and so is my CCM. She is the bestest. I'd suggest you do the same. It needs to be determined if it is indeed one auditor and their preference rather than what is heard. Man, if I knew that we could use words simply because it's our preference I'd be using CAN'T, DON'T, AIN'T et cetera! (well, you know what I mean...)

I have always typed PT/INR just as you did and so far have never been called on the carpet about it. As for dorsosupine, there is no such word. It should have been "dorsal supine" just as you typed it. I don't blame you for being upset about it. BTW, I am a QA person myself - in training anyway. I certainly would not have flagged you for those things.

Sure it nice to hear from all of you who feel I am correct. Seems like whenever any of my reports are audited they just HAVE to find SOMETHING to ding me on. I even got dinged because in PE would do the heading and leave out word like "are" in LUNGS: (are)Clear to auscultation. JEEZ. I would LOVE to know how they find enough time in the day to audit any reports from India. Would take me all day just to correct one I'm sure. Also, another thing I got dinged on was typing "second" portion of duodenum instead of "2nd". DOUBLE JEEZ.

In your example of using "are" after the heading LUNGS:, you are correct in leaving it off. It is a grammatical rule not to use a verb after a colon. You can prove this one using the BOS or a Gregg's manual of English grammar.

Well, if dorsosupine is a word, then it is not in my Stedman's Medical Word Book and it is also not on google MT. Tried to look it up. Maybe old Claudia doesn't know everything after all. LOL.

I would challenge dorsospine. Since when is the "rule" to combine an adjective with a noun? Dorsal modifies spine. As for Claudia Tessier .... not an authority.

Well, sent another email about dorsosupine and PT/INR to person who audited me. Got a long convoluted message back about how "dorsosupine" is in Tessier book (guess she's a better reference than Dorland's) and that PT/INR is DEFINITELY wrong (got into chemical descriptions, etc. - didn't read the whole thing to tell you the truth) and it should be PT INR. Oh well - guess my 35 yrs in the business doesn't mean much anymore. Funny that whenever I really get stumped on something NOBODY can ever fill in my blanks sent to QA, but I'll get picked on for these things just to have some sort of a "shadow" on my record even though I've been declared one of those "account trustees" in my BOB. What a joke.

and I can't believe you got dinged for PT/INR. I have never seen it expressed any other way. Sounds to me like your QA person has no medical knowledge.

The combination form dorso- cannot be used with supine, which is a position (and not a direction like dorsolateral or dorsomesial). It's the same concept as neural foramen, which also cannot be combined.

I had a virus that kept closing DQS and I would get pop ups while I was typing. It also f-ed up my spell checker. After 2 weeks of this support finally decided they would send me a new computer. I got paid for 6 days downtime which is good, but I had an audit while I was in that whole mess and failed one report with a 90%. I guess the spell checker was mixing up my words when I corrected them. It was changing things like anemia to enema, stupid stuff... but I got my ass chewed out for it and I am going to get audited again in 7 days to make sure I am just not a horrible MT. She told me that I should have went threw each document with a fine tooth comb. I told her that I can't afford a fine tooth comb because my paycheck was so little. That totally pissed her off. I didn't mean to sound all smug if I did, I was just frustrated I spent 20 minutes on the phone with her telling me the same thing over. I assurred her that now I have my new computer there should be no problems and she will see me pass the next audit in a week.... so then she started repeating things she already told me like I was an idiot. I admit I was a little smart ass after 30 minutes on the phone.

You're absolutely right on target about ending the conversation! The part where you would have asked her to email you with anything else, though, should have taken place at the beginning of the conversation. A person should get any of that sort of accusation IN WRITING!!!!! That way, you BOTH have a copy of the accusation, and YOU PERSONALLY are in a much better position to defend yourself. Not being rude but I noted that you have some misspelled words in your post - i.e. threw (should be through) and assurred (should be assured). These really stood out to me.

Maybe you should check your spellchecker program to be sure it is correct although threw would not have been caught that way either.

well that happens when you are typing in a hurry. I don't get paid to type on this board and have every little word correct!!! You get what you pay for...lol... I get nothing on here so I don't check my posts. I barely get anything at MQ, but that is where you can literally be sued for being wrong, so I check there....

That is very unprofessional! She should not be a manager. You cannot speak to people that way. She is definitely in need of performance management! If this happened to me, I would absolutely take the problem to her manager (the regional director).

You had documentation to back you up that your computer was not working properly. Take that documentation, the email from you CCM and a description of the phone conversation and report it to the regional manager, or to someone on the contact list on MQ Central.

It is inappropriate for anyone you come in contact with on a professional level to call you names. You don't have to take that, you deserve to be spoken to with respect.

Can somebody tell me what this audit/QA procedure is now?  I'm so confused with when they are done, how many reports are done, etc., etc.  I had been told earlier that 5 reports would be randomly pulled monthly and checked.  You would be notified you had been audited and the results.  If you passed, another 5 would be done the next month and so on.  If you didn't pass the audit, then a redo audit within 7-14 days would be done on another 5 random reports.  ...

I don't understand how auditing works.  Two of my reports were reviewed, and in each case, a word I had puit in had a line through it, but then it was replaced with the same exact word!  For that, I got marked off, getting only 99.50% or 99.75%.  I wish there was a way we could give direct feedback to the auditors at the time we're viewing our reports. ...

:(   I have tried to give the QA program the benefit of the doubt, thinking it was being done fairly, rewarding us for near perfect QA scores.  I have managed to keep my QA score above 99% every week until last week.  Before last week, I normally have 8 to 11 reports pulled for audit, some short, some long.  Last week I had 1 critical, 1 major, and 2 minor mistakes which dropped me down to no bonus pay.  Not faulting anyone for that, totally my mistakes, legitimate errors.  But....once th ...

I have only had 3 audits in 2-1/2 years. Even though I received notice of scores of 99.6 or 99.7, I did not receive any account of what my mistakes were to produce those scores. A comma? A hyphen? Okay, I may be the last one to know this, but I thought the only QC audits I was getting were the ones showing up on the DQM QA Feedback screen where it says (1 unread) or (how many ever unread). When you click on them, if the report has been audited by your QC, it will have a percentage score.  But that's just for the reports you sent in with a blank. This is silly Not only do we need to follow CP guidelines but BOS and then DocQscribe standards. I got CRITICAL errors on audit for things like:  I typed "I used a translator telephone..." (that is what I heard) and the person auditing changed "a translator telephone" to "the translator telephone" (I suppose she heard "the" instead of "a"). In the physical exam portion of one report, the dictator used "was" throughout except for one time he said "is" and so I changed it to "was" to make it consistent and got a critical error for that. Please tell me how these are critica ...

Does it seem like no matter what you do it is not good enough? Here lately it seems like they only audit the worst dictator. The only reports they have audited for me lately have been ESLs lately. Maybe that is their way of trying to get rid of me. Maybe they want to thin the herd and just send it all to India. I just hope to find something soon outside of MT. I have been looking.   ...

I'd like to say I have had to learn accounts on my own and have had to cross reference many, many times.  Yes, I have seen blanks where if someone had taken the time to hunt for the word, it would have helped me also, but for the most part with what we have to work with, I have seen words that are highly inaudible by many, many doctors with bad dictation and yet what was typed was a wonder we could make anything out of it.  I have seen no help from the hospitals themselves as ...

Just wondering if anyone else has had this happen lately.  I received a quality audit on 4/25 on my audit, which I passed at 98.3% (had one icky one in there).  Then on 4/29 got another email of an audit of 5 reports, did not give a total score, but they were all pretty darn good (3 at 100%, 1 at 98, 1 at 98.75).  Then on 4/30, another email, they checked 3 more reports (all those above 98% too).  But what is going on, are they searching for something bad?  Are they doin ...

I am NOT going to read all those posts, but I just wanted to let you know that you are lucky you only get audited once a month.  I get audited DAILY, at least once a day.  Today I got audited twice, one 100% the other 99.75%.  If I can remember correctly, I got dinged for typing something like "Aorta was tortuous" instead of "The aorta was tortuous".  It was for an account that has a multitude of dictators - I swear there is a new one every day and no two on a consistent basi ...

I don't usually post here - guess I've reached my discouragement limit for the night! I work on a very difficult account as I'm sure many of you do as well, and I spend a great deal of time listening and re-listening to these difficult dictators to make sure I am hearing correctly.  I just checked my audits again and see that I have received another 1 point deduction on a post audit for something they heard differently that I apparently did.  It frustrates me no end that ...

I want to repeat this for those who may have missed it and I am not looking for pity.   Was moved to a new account after 4 years on 2 very difficult accounts with only 1 or 2 failed audits (and was always given a second chance).  After 24 hours on the new account and telling them there was one particulary dictator I could not hear because he kept moving away from the phone, they audited me, included reports from that dictator, and immediately put me on 100% QA at which point I finally ...

I have yet to pass one, how the heck, with say 3 minor errors can you possible pass these things? I want to quit so badly but have no prospective jobs. So the question boils down to basically, is at what percentage of errors are they firing people? 99.5, 99 what? I just cannot see how this is possible to do. Aren't we supposed to be getting these audited reports once a week? Ambulatory MLS just started using FIESA recently, so I have only received 1 corrected report so far, but that was back on 12/3.  I haven't gotten anything since then. Is the QA% on that report what I'm supposed to use to figure out my pay for this week or should I be receiving something else? Thanks...  ...
