I hate "qualified verbatim" so much. - How do you guys handle it?

Particularly I never know what to do about headings.  If they never dictate headings, it is no problem - I do not put them in.  But if they dictate no headings, until all of a sudden towards the end of the report, they dictate one random heading, then it makes the rest of the report look stupid without headings.

Would you put in headings for continuity in the rest of the report, or leave it looking stupid with, eg, one single heading in the middle of the report? 

When I look at past reports, there is no consistency in how people handle it.  Would appreciate some opinions.

The definition of "qualified verbatim" in the guidelines is so darn ambiguous. The whole concept annoys me anyways, like the client does not trust us to use our judgment. I can see the value of QV maybe for speech rec accuracy, but somehow I do not think that is why clients request qualified verbatim.

We have quite a few, in fact an entire department for one facility it seems, who've decided that they need to be on QV. The way I interpret the ambiguous instructions is this: Other than uhm, uh, and the like I type it as they speak it. If it's wrong, it's not my fault. If it's weirdly formatted, that's on them. I may leave a note to the client if there's a very strange statement or value, but otherwise it goes as they say it. The only place I "add" punctuation is for dictated lists - 15 items and no commas, well that makes me twitchy. Otherwise, most of my QV cases like to give their own punctuation, which is usually weird and wrong. Oh well.

the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested."  I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..."  So I decided to omit crappy and just say "she is still sounding pretty congested."  QA w ...

I am under the impression that QT is canned text that we bring into the report and add text at certain spots when the dictator specifies? I used to do a lot of ophthalmology reports. Surgeon would say: Please insert my normal phacoemulsification with IOL implant. Then they would dictate which eye, how many diopters, model and size of lens, etc. Does QT also sometimes just show up on the report that comes up and then we just add what is dictated? How long does it take to bring in the QT? Than ...

Question About Nuance Policy On Qualified (template) Lines

I was originally with one of the ambulatory groups that came through the back door of Nuance.  Our old ways of doing things have slowly been phased out since Nuance acquired us.  Currently I get 8.5 cpl for standard lines and 1 cpl for qualified (template) lines.  I haven't gotten the email about the video yet, but I've been reading on here about the new 7 cpl rate. This is almost/sort of a poll. I need to know what verbatim means to you in VR. Do you change any wording? My understanding of verbatim is word-for-word (Merriam-Webster dictionary) unless a sentence does not have proper structure; if the doctor rambles on and puts the word 'and' between words where there should be a period, etc.  (Not included in this is changing mg to mcg in a wrong dosage, changing the medication itself if wrong, or other major medical erros&nbs ...

Is it me or what - I was always told to follow account spec and make sure I didn't use the forbidden JCAHO abbreviations. I have all verbatim-verbatim accounts and only just now learned that we have to change what the dictator says and change it to an "approved heading."  I was once told this only pertained to non-verbatim accounts, but was just corrected.  I was told that ALL OF THE CHECKED headings in the headings pane must be included in the document.  If the doctor says procedure, you must put in an approved heading into the standard changing what he says if in the approved headings list it says ...
