First of all, there is NO such thing as "verbatim." I just cringe every time I am told "verbatim." Notice that each time you are told that an account is "verbatim" that a "but" always follows it..........either it is verbatim or not. These companies need to look up the "verbatim" in the dictionary......sorry just my opinion.
I'm not sure how I would transcribe that. I might be inclined to type it exactly as I heard it, as you have.

Sorry Shortcake I did not mean to offend. It just seems funny that when an MT is told the account is "verbatim" it never ever is.....if it were could you only imagine? Actually, I think it would be quite fun to just 1 day be able to type it all "verbatim." So I am sorry if you thought I was being mean, I really wasn't.

It's funny that you can't change an adjective to an adverb when called upon grammatically or proper verb tenses, but you can change a lot of other things, strange things at that. I took your intended meaning from the very beginning! Amen!

I am so glad you understood what I meant. I honestly would not deliberately be mean to another MT about something like that.....but doesn't it just make you cringe?! I mean have these clients/companies bothered to look up the meaning of "verbatim.".....I have been an MT for going on 20 years and when I first started out we were taught to "make the report read." We never had any problems.....now.....ugh! Have a GREAT day!

It would be wonderful to have one day when we could type it all verbatim.
I do believe many docs think our job is effortless, since our final product is completely understandable.
If only they knew how many times we saved them from syntax suicide!

I'm trying so hard to keep the blanks down, which my % is really okay, but I'm finding more and more that these docs are just not making any sense and the mistakes are getting ridiculous!!! the doctor is dead wrong about his anatomy? The patient has a problem with his foot and leg, but he dictates ulna. I wouldn't give it a second thought to leaving it blank, but I already got notice about documents with blanks (usually 1-2 blanks if any, and anymore than 3, to QA). Hmm... go with what is right or what is the least path of resistance (i.e. least likely punishment)
BTW - The document I sent yesterday with 4 blanks to QA, they couldn't fill them in either. Shesh, the ...
The reason I ask is because this morning I'm doing an acute care progress note where the dictator says "The patient has end-stage chronic obstructive pulmonary disease and she is still sounding pretty crappy and congested." I absolutely refuse to use the word "crappy" in a medical document if it is not within the context of a direct quote from the patient, i.e. "I feel pretty crappy..." So I decided to omit crappy and just say "she is still sounding pretty congested." QA w ...

I have all verbatim-verbatim accounts and only just now learned that we have to change what the dictator says and change it to an "approved heading." The dictator clearly dictated a word with an apostrophy following it. The word actually has an accent mark following the last letter, but he specifically dictated an apostrophy. I got it marked as an error.

I'm on accts that do not allow any blanks at all through to the cliient. ALL blanks must go to QA. I haven't ever worked on an acct that would not even allow one blank to the client. Anyhow, might not would be so bad but the dictators are TERRIBLE! Many ESLs also.
Combine no blanks allowed with terrible dictators = sending more to QA. That means docked pay if 5% or more. Oh, and don't let me forget, no samples yet. So you have to ho ...
