how to get my speed up when i get different docs and specialties every day - kacki

I do transcription for a hospital conglomerate, and we have thousands of doctors. I find that ever since we started a new program last year, I no longer get a lot of the same docs like I used to, but I get different dictators, different work types and different specialties all day every day. It's very frustrating because I find that I'm learning new specialties all day but, because there's apparently nothing I can do about it, I have to just live with it and work on getting my speed up regardless. Right now I'm only at about 320 lph, and I really want to get it up to at least 500 lph. We have Editscript -- can anyone out there give me some tricks on getting my speed up regardless of getting different docs and different specialties all day? Thanks!

That isn't bad considering the amount of docs and different work types, etc you have. Editscript is a great platform. I want to get a job where I can use that platform again. I hate Futurenet, doesn't even have a medical spellchecker on it.

really? i'm being led to believe i should be able to get my speed a lot higher. honestly, though, the problem really is the different docs/specialties all day long. i know a lot of other people where i work who get the same docs and same work types pretty much all day every day, but i get brand new docs every day, new specialties every day, and some days i get so frustrated i take breaks often throughout the day. can anyone give me an idea what it's like other places - do you get a lot of the same docs and specialties and work types?

I don't understand why most companies do this. It not only hurts your production as an MT, but you end up sending more blanks to QA, which costs the company more and keeps the report off the chart longer.

At my previous job, I worked on a hospital account, all different work types and LOTS of different doctors. When I told my boss about one particular area that I liked (that nobody else liked because they were ESL), she was happy and started assigning all those jobs to me. Win-win for everyone.

I was lucky to find the job I have now. Everyone has doctors assigned to them. I have 2 docs and I'm the only one who types their reports. I can make templates for all their favorite phrases and I can fly through those reports! I know this isn't for everyone; some people would be really bored with typing the same stuff all day long. But when I look at my line counts at the end of the day, I have no problem with it!

Maybe you could say something to your account manager. Let them know that by narrowing your work pool, you could be much more efficient and accurate, two things that they should appreciate. Be sure to let them know which work types and/or doctors you are good at and see if they will accommodate you. Hey, couldn't hurt to ask, right?

you are so right! it hurts my production AND i feel like i send more than i should to QA. i actually went to my manager and mentioned it to her last year, but she insisted it's "random" (which cracked me up -because the work is ASSIGNED which, of course, is the OPPOSITE of random). i communicated with a lot of people who all said they get the same docs. i just really don't know what to do any more. i'm afraid to say anything to any other managers about it at this point. would you consider telling me what co. you work for? i'd kill for the same docs all day! i know i could get my speed up a lot if i didn't have to get used to new docs and look up new terms and reports all day. help!

Lately my problem has been focus, but I can average around 250 to 300 though, have trouble maintaining 300 on a consistent basis; 225 to 250 is the norml

I work on Editscript and I can do 500 lph under the most fabulous ideal circumstances but it cannot be sustained. Feel very, very pleased you are doing 320. I average 200 and I have been doing this for 15+ years.

wow. that surprises me. i was told at a recent f/u training meeting that i should be able to get it much higher. about the new docs and new specialties all day - does anyone else have this? i know that at our co. others have told me they largely get the same all day every day, but then there are those of us who get diff docs/diff specialties all day. it's EXTREMELY frustrating - i can never get used to anyone 'cause they're usually different. anyone have any advice about getting speed up in this case would help me so much.

It is unheard of to do 320, much less 500. She may be getting confused and doing VR at 320, but even that is about unheard of. Every time I see a post that says they do this outrageous amount, I just shake my head. Highly doubtful

um, i'm not "confused" at all. it IS voice rec, and my latest was over 350 voice rec for editscript. in fact, we had a re-training recently, and i was chastised by the trainer when he saw my lph was 282 (i've gotten higher since then) - he said i should be doing better. he even said they have people who do between 600 and 900! i just wanted to know if this is realistic w/voice rec in editscript - especially since i get 100 new/different docs this week - and then 100 new/different docs next week! i just wanted to know if a scenario like this is typical, in which case i'll just shut up and do it. (although i have spoken w/others where i work who say they get pretty much the same docs.)

If VR was 90% accurate, maybe you could do that but the reality is that VR is maybe 65% accurate.

Based on my own experience doing VR and working on Editscript (which is the best ASR IMO) the max would be 350-400 and that would be if you were seriously disciplined and worked 8 hours straight with zero distraction.

Go like the wind! Leave blanks immediately if you dont hear something. At end of report, quickly give a relisten, if you dont hear it, get rid of the report with the blank in it.

Nobody cares anymore, so you have to stop caring so much. It wont hurt the doc to have a blank, and it wont hurt you to leave one.

Yes, we are perfectionists, however, they changed the game, so we have to change the way we play.

Why would you give that kind of advice this MT? Do not lump us all into one group. There are a lot of excellent MTs out there that still care about quality and doing the absolute best job they can do.

My advice to you kacki is to make a TON OF SHORTHAND. Make sure that it is for all words not just medical terminology. This will help you increase your speed without compromising your quality.

Give me a darn break! If the hospitals cared, they wouldnt offshore or go for the cheapest work possible. If the MTSO cared, they would give you a primary account. If ANYBODY cared, we wouldnt be having this conversation!

Prance around and spew how great you are and how much integrity you have for your whole 4 cpl. That is a contradictation in itself, is it not? If they expected perfection, we would be paid accordingly! Time is money.

trying to figure out what a dictator is saying. And what I mean by that is below.

Many doctors are being overburdened with too many patients and rush through their dictation for that reason, and a few because they lack probity and are negligent toward their patients.

On down the dictation chain, the providers, often the physicians' employers, refuse to pay our MTSOs what our work is worth but insist on quality. Our MTSOs accept the low contract and pass the demand for high quality on to us, along with the low pay.

What all this amounts to is that additional production COSTs in terms of time to produce high quality are passed on down the chain to us, the people at the bottom. Those who work extra diligently to fill in all the blanks--instead of sending them back up the chain to be filled in by the author--are assuming those costs of production, but none of the profit being generated.

Like many others who've been around a while, I have a lot of difficult dictators assigned to me because I'm more successful with them than most. Note that I am NOT given more leeway in how many reports I can send to QA and still qualify for bonus. I am not paid a higher line rate for doing them. If that's not bottom-of-the-chain enough, most of the bad dictators are now processed through SR, whether it can handle them or not, so I get half pay for them.

Thus, I also try to "fly" through my reports--very accurately, always remembering each report has a patient's wellbeing depending on it, but I do try to limit the costs I'll assume by filling in all blanks from my position at the bottom.

I attempt to fill in all blanks by relistening, checking other reports, etc., but I do this in a "smart" way. I do the entire report, dropping blanks quickly as I fly (as "SM" suggests), because the answer often turns up below and because I just "get" the dictator better after listening longer.

In filling the blanks, I choose those most likely to be successfully filled and give up fairly quickly when an answer is not found in the first couple of reports checked. My company says check only 3 (trying to stop us from searching them all). I feel 2 is duly diligent these days and feel stupid when I find myself searching the entire hospital course--which I do too often because I do care.

I stop when I've filled all I'm likely to AND when the number of blanks falls below the requirement to send to QA. I'm pleased when a chronically unintelligible dictator doesn't have to go to QA as usual but don't regard it as a personal failure. Because it's not.

Nor do I regard sending "feedback" of blanks to be filled in by the person responsible for them--the author--as a bad thing. I think of it as informative feedback for the good ones and a poke with a stick for those who don't care that much quality but do dislike being bothered with corrections.

am dedicated to quality (have been praised by my MTSO for such) and if SR renders something unintelligible or dictator mumbles, etc., etc., I have no problem leaving a blank and moving on. I do give it one more listen when I'm finished but that's it. Our pay is so low now that time is indeed money. I'd love to spend all the time in the world researching, but even my MTSO tells us not to. The game has changed and as much as it pains me, I have to move along or I'd be making less than minimum wage. This has nothing to do with lack of integrity, it is now a matter of survival. That is just the reality of the situation.

I too have a ton of integrity. My supervisor told me to listen 3 times and if I don't get it clearly to leave blank and go on. Yes the game has changed, but I have to also pay bills. So I try to find a happy medium. Give a decent try, then let it go. Even the book of style says when in doubt to leave a blank. ;)

actually, it's hourly plus differential for working later hours and then incentive program on top of that for production over 235 (voice rec) and a little higher over 170 regular. it's a great place to work and i LOVE my schedule. that's why i asked if anyone could help w/the too many docs thing, getting my speed up regardless of getting too many new docs and specialties.

it's a giant hospital conglomerate, and yes, i do feel blessed to have such a good job. i love it there and hope and pray it never goes away! i've stepped up how hard i work lately because i just want to make myself more valuable, since i have virtually no seniority - only been there 2 years (and w/hospital for 4). some gals have been there 15 plus years! there are probably online companies that pay more, but i like this job. if it ever gets outsourced, then i guess i'll look for another job. unless or until that happens, i'm staying put.

Just not gonna happen. They all pay based on production and if by chance you do stumble across one that pays by the hour, you can hang it up with regards to incentive - It does not exist. No company will pay the kind of incentive you make in a hospital.

Also now a days the companies standard pay is 8 cents per line for straight typing and 4 cents per line for voice recognition. Some pay as low as 3 cents per line for voice recognition. And for straight typing there some who only pay 6 and 7 cents per line. It's terrible.

I worked for a hospital like you for many years before getting outsourced to a company. I took the job and luckily for me I make 6 cents per line for voice recognition and 9 for straight typing, but that is rare anymore so I consider myself fortunate. Stay put because jobs like yours are a rare commodity in this day and age. Good luck getting up to your 500 lines per hour.

Oyyy...where to begain. It's sad that so many of us are looking for second jobs to supplement our income when the board members and powers that be are making beaucoup $'s. No one should have to sit at a computer for more than their shift to make a decent living!
Regarding increasing LPH, there is only so much info you can add to a program and only so fast a person can work and put out quality work! I was so disgruntled and began to hate the job that I have loved for 25+ years. My husband noted I was not the wife he married. My parents noted I was not the bubbly daughter they knew anymore. I found myself cursing at the computer all the time and stressed out. We were getting emails with suggestions to do more, read more, train more (on our own time, of course) and still people are being hounded about their LPH. I began to feel like an emotionally, verbally and mentally abused wife! I am fortunate that my husband is very supportive of me, recognized the abuse and attacks by the company I worked for and asked that I quit my job for my own health and mental well-being.
It seems the mighty dollar to the board members and CEOs are the main priority. They used to hire only people with 5 years or more of acute care experience and now they are not so picky. My feeling is, they believe there will also be someone else to take the job that someone vacates. What I don't understand, having worked for a wonderful surgeon at the beginning of my career, and his surgical center, is that when you treat people well and provide a decent income for them, you don't spend more $ in training new people, using QA more often, etc. These companies are making no business sense to me at all.
Also, when is the US Govt. going to recognize the patient information is being sent to foreign countries and that information can be used in inappropriate ways against our own citizens? Isn't this a threat of some sort to national security (gaining passports with the info, etc.)?. Am I wrong in seeing this as a very bad idea and a major loophole for people to get into this country with fake passports. Do you think HIPAA means anything overseas or that our govt. will be able to protect any patient against HIPAA violations, fraud, scams, etc? This worries me and it also worries many politicians and doctors who are unaware of such activity. I can't do it alone, but I have been raising awareness in my state, informing our representatives and citizens about what is going on with their personal information. I don't want to sound like "Chicken Little" but I do believe there are going to be some major ramifications if this continues, if it's not already happened. This is only my personal opinion on the matter. I am thankful that I am financially able to take a stand and not work for people and companies I don't believe in, treat people poorly and/or put US citizens at risk of their data being stolen. It's always about the mighty dollar and padding their own pockets. They can spew it being cost-effective, that they are filling shifts that no one wants to work, etc. but I see people who are willing to work evenings, nights, weekends - just look on the Job Seekers part of this board. Another thing, how is it cost-effective when (as I have heard from my manager) the work coming back from India, or wherever they decide to send it, has to be re-edited? That does not make sense. How can it save time or money?. Just my thoughts. I am trying to be positive now, but I have such a bitter taste in my mouth during the last 1-1/2 years of being made to feel like I should do more, more, more. Also, I carried a QA consistently over 99% and had no vicious emails regarding my work and I STILL felt abused. God Bless!

always the same pace and routine of same way for each report just use SH with all words

if you do this you will make significantly more lines, thus significantly more money period.

Had an appt with my doctor; she was slowly typing as we talked.  I asked her how she liked typing her own records.  She said she hates it but has no choice as it is required now.  Apparently some companies are telling the hospitals that EMR is required by law.  That isn't exactly true, and the companies (such as MQ) do a lot of fibbing and twisting of the truth.  While it will be required that medical records be electronic....HOW they get there is where they are fib ...

How can we edit/transcribe if the volume is highest on the computer, ear phone amplifiers are on, and we STILL CANNOT HEAR!  If it comes straight typing and ASR cannot pick it up, why in heaven's earth does the doctor think I CAN HEAR. VR - Docs Almost Say A Word

On VR, many times a day I hear part of a word like "s" which might be "it is" but if I add that, I might get a Nuance error.  But if I don't add it, it would be an error.  Depends on the QA person.  You really can't tell when listening to it.  This is multiple times a day!! Night after night I pretty much do the same doctors (not that I am complaining about that), but sometimes I get frustrated because I am floored at how these doctors can get through medical school and NOT KNOW how to pronounce medical terms and medications!!! AAMT, the industry, everyone says to be perfect and get everything correct.  HIPAA this, and HIPAA that.  If we are held to that standard, how about the doctor? I have an account I work in where the work is mostly the same doctor for the whole shift.  Seems lately my hours coincide with Dr. Never-shuts-up!!!  It's not uncommon for him to dictate 10 + minutes for gastroenteritis!  Yesterday he did an 11 + minute note TWICE on the same patient.  What do you all do when all you want to do is SMACK the doctor, lol?  ...

A bunch of cackling hens in the background of an already mumbling dictator is just ridiculous. Why do they not just tell them that phones pick up what is in the background? They now cut our PTO and are threatening us if we don't have all our hours in because THEY are out of work..Oh yea and at least 150 lph and 99.6% is it now accuracy..When I can't understand a doc now, I will not go in past reports, I will leave markers..I will go for a second listen once the report is done but the old Above and Beyond crap is gone!! ...

I've only worked on the new 6.1 for 3 nights now (don't like it, I could go on and on), but this takes the cake. Strong ESL doc.  The doc in the report he refers to I can't understand, but I can tell it starts with an "L" or at least I think so... So CTR N and I type an "L"... Just typed a letter to a peer review committee regarding a discrepancy in an operative report and a discharge summary.  The doctor gave the explanation that it was simply a typographical error either from his mumbling or unclear dictation, and that the dx listed in the discharge summary did not make sense and he would not dictate that. Yes, he actually admitted it could have been from his mumbling.  But, why didn't he catch that incorrect diagnosis when he signed the discharge sum ...

Platforms That Don't Allow You To Open Word Docs...

I've heard that some MTs use Word Pad for their notes, which can be opened in platforms that you can't open Word docs with.  I just got a new PC with Win 7 and whenever I save a WordPad doc, even specifying RT format, it saves it as a Word document.  Do we have to use Notepad for notes to open while working or is there a way to use WordPad to save in other than .docx format?  I changed settings in WordPad but it still saved it as docx.   ...

I have been here with MQ 4ever it seems.  Today, I just need to rant.  It seems that for the last week or so ASR has gotten WORSE, leaving out words and still not changing terms which have been repeatedly "corrected" by me.  It is not "learning".  Of course, I guess this means job security.    But, the next rant I have is how many ways can someone enter a docs credentials and initials in?   Constantly leaving out the periods after the middle initials ...
