Can anybody PLEASE tell me the correct way to put a patient's last name on the 1500 form for a Medicare patient when the last name has three parts separted by spaces, i.e. AA BB CCC, FirstName, MI, which is the exact way the name is on the patient's Medicare card. People are not doing a full listen/correction. There are some pretty blatant errors, and these errors are being repeated by subsequently dictating physicians who are obviously using other documents for information (as it should be).
