Is it too much to ask for the docs to SPEAK UP - MTannoyed

How can we edit/transcribe if the volume is highest on the computer, ear phone amplifiers are on, and we STILL CANNOT HEAR!  If it comes straight typing and ASR cannot pick it up, why in heaven's earth does the doctor think I CAN HEAR. I cannot!  Are these docs being passive aggressive... Oh they don't realize?!  That is just MEAN, CRUEL.  Forget that I have to make a living wage, but think about just plain RESPECT.  

This vent brought to you by me getting too many dictations in a row in which the doc is whispering so quietly nothing can be heard. And I mean nothing.

I also have had numerous MDs who sound like they are dictating 10 feet away from the microphone doing God's knows what. Actually, I think the MDs have the microphone up their butts instead of in front of their face when dictating.

Where their dictation is obliterated by loud breath sounds who obviously have never listened to their dictation.

The first time I had one at an extremely low volume I walked into her office to see where she placed her recorder in relation to her mouth. She was holding a cassette recorder speaking directly into the SPEAKER instead of the MIC!

I complained over and over, yet the problem continued every day. MTSOs will not communicate with their clients about the problem UNLESS they think it is a technical problem they may be blamed for, or there is a chance the doctor will have to redictate the reports. This cuts your productivity in half when you have to listen over and over and over.

If you complain to your supervisor that there must be a technical problem that will require the doctor to redictate all his reports, they will take action. No MTSO wants to inconvenience a doctor! Plan B if that doesn't work is to explain that you could produce twice the number of lines per hour if the problem were rectified.

Back when we had our own clients, all we had to do was sent the doc a note asking him or her to hold the mic closer or speak up which always solved the problem for me. I think these doctors don't even know how they sound.

It all started approximately a month ago with a note from CCM saying she was going on vacation.  Not a word from her since then. Letter from corporate saying I had a new CCM.  Wrote that person and got a reply that she would like to help me but I wasn't hers! Been well over two weeks since any word from QATL. I was on lockdown, now I'm not. I'm still on the same old accounts for the most part, with the same old CCM listed, but nobody seems to know it.  LOL Anyo ...

I just put some things together that are enlightening.  MQ/Spheris has 12,000 MT's in the U.S., not counting those overseas.  Transcend has 5,000 MT's in the U.S. and another 1500 in India.  Webmedx has 1200 MT's.  KeyStrokes has 600 MT's.  TT has 200 MT's.  Softscript has 200 MTs.  NEMT has 200 MT's.  Transolutions, or whatever they are called now, has 300 MT's.  Medware, again not sure what their new name is, has 3 ...

I mean seriously this catering to their stupidy is starting to get old. Granted they aren't major errors. But, my theory is if they cannot give me the courtesy of even remotely completing a sentence, much less a paragraph here lately, I am sending it through. I'm trying to get an answer from my CCM regarding foreign doctor's dictation and this 5% QA submission. I type for two separate teaching hospitals and are constantly getting new foreign doctors, 3/4s of which cannot even speak English. I won't matter if I got copies of every single report they dictate, there would be no way not to send these reports to QA....are we getting any kind of special consideration towards our QA submissions for these doctors? They expect us to understand their Vietnamese, Chinese, Arabic, Indian, Filipino, Hispanic accents when they can't even put a simple English sentence together!  The hospitalists are the worst.  I think they hire them right off the boat from their overseas countries and put them to work in the hospitals-- probably as cheap as they can get them, and then put them on VR.  My MTSO increased their line requirements for this year and with over 30 years' experience, I struggle ...

MQ told us at one point to use that ponderous webmd.com website to find docs. In my state I use the state medical assn website to find docs. Had an appt with my doctor; she was slowly typing as we talked.  I asked her how she liked typing her own records.  She said she hates it but has no choice as it is required now.  Apparently some companies are telling the hospitals that EMR is required by law.  That isn't exactly true, and the companies (such as MQ) do a lot of fibbing and twisting of the truth.  While it will be required that medical records be electronic....HOW they get there is where they are fib ...

People are not doing a full listen/correction. There are some pretty blatant errors, and these errors are being repeated by subsequently dictating physicians who are obviously using other documents for information (as it should be). VR - Docs Almost Say A Word

On VR, many times a day I hear part of a word like "s" which might be "it is" but if I add that, I might get a Nuance error.  But if I don't add it, it would be an error.  Depends on the QA person.  You really can't tell when listening to it.  This is multiple times a day!! Night after night I pretty much do the same doctors (not that I am complaining about that), but sometimes I get frustrated because I am floored at how these doctors can get through medical school and NOT KNOW how to pronounce medical terms and medications!!!
