General Word hel p questions about ESL docs - Confused

New to ESL doctors... wow fun.

doctor is saying microcytic or macrocytic anemia.  When you have a word that has one letter difference, do you always send to QA?  I am doing my best to look up which is what, but I am not going to guess.

I just hate that my QA maybe thinks that I have no idea what he is saying, because we aren't supposed to write S/L in our box of notes to QA... the QA thing is very new to me and I am having a hard time getting all kinds of corrections that in my previous 10 years were never problems!  ugh.

Macrocytic anemia is a B12 deficiency and microcytic is just an anemia. I often use the context of the rest of the report when determining.

On ESL dictators, sometimes its very hard to determine whether they are saying microcytic or macrocytic. Hypoglycemia vs. hyperglycemia is another big one.

I have 30+ years experience and do exactly like you. I will try my best to determine which one it is. Sometimes the lab section can be very helpful, i.e. if the patient was admitted for hypo/hyper glycemia and in the lab section the patient's blood glucose is 800, then I know that it's hyperglycemia.

Otherwise, if I can't tell and can't be 100% sure, I always leave a blank. Sometimes the ESL dictators will even realize the problem and are actually nice enough to start dictating "hypOOOOglycemia" or "hypERRRRglycemia."

On your QA corrections, my guess is that you're probably working for a company that uses the BOS (Book of Style), which maybe you never had to be concerned about in your past 10 years. Personally, I think that a lot of the stuff in the BOS is a bunch of hooey that doesn't affect patient care at all. But my company also uses the BOS, so I'm stuck with it too unless I want to get marked down by QA.

and You are exactly right about the book of style (and now the changes from the 2nd edition to the 3rd even!) has nothing to do with patient care. I did follow some of the rules before, the ones that made sense. haha. I understand wanting to be the same as everyone else, but missing a dash, or putting one in... oh man it's killing me. little things like that. I'm not missing words, I'm not misspelling things... I'm not getting anything other than these stupid BOS things wrong... I just started with the new company 3 weeks ago and there is a lot for me to try to learn and remember, especially after doing it one way for so long. and some of that BOS stuff is just HIDEOUS... IMO :) but oh well, I am trying!

I went through the exact same thing. I've been an MT for a long time and about 5 years ago I started working for a company that uses the BOS, and are BIG sticklers on following the BOS to a T...right, wrong or indifferent. And QA couldn't find anything wrong with any of my reports, except for those pesky little BOS things like the hyphens.

And then you've got things like the BOS2 says to hyphenate metric measurements like "10-mm cannula." But then you go over to the BOS3 and THEY CHANGED THEIR MINDS, so now it's supposed to be "10 mm cannula."

Platforms That Don't Allow You To Open Word Docs...

I've heard that some MTs use Word Pad for their notes, which can be opened in platforms that you can't open Word docs with.  I just got a new PC with Win 7 and whenever I save a WordPad doc, even specifying RT format, it saves it as a Word document.  Do we have to use Notepad for notes to open while working or is there a way to use WordPad to save in other than .docx format?  I changed settings in WordPad but it still saved it as docx.   ...

If you have a doctor that dictates a procedure, very clearly, and you can actually spell it, but it comes up misspelled in spellcheck. So I go on google, look in all my books, and benchmark (can't help it I am anal) and cannot find that exact procedure. In this case it was dictated as an "otomy" but all the references had is as an "ostomy" do you leave it as "dictated and spelled," do you change to the correct form which is the "ostomy," or do you leave a blank? If you do general transcription, can you recommend a good (free or low cost) transcription program for audio files?  I tried InqScribe and it works okay, but I don't like that they make you use their built-in word processing program that doesn't have any text expanders. I use Express Scribe.  Do they have a way to transcribe videos?  (I think it has something called Prism video, but I never figured out how to use it).  Thanks. ...

MQ told us at one point to use that ponderous webmd.com website to find docs. In my state I use the state medical assn website to find docs. How can we edit/transcribe if the volume is highest on the computer, ear phone amplifiers are on, and we STILL CANNOT HEAR!  If it comes straight typing and ASR cannot pick it up, why in heaven's earth does the doctor think I CAN HEAR. Night after night I pretty much do the same doctors (not that I am complaining about that), but sometimes I get frustrated because I am floored at how these doctors can get through medical school and NOT KNOW how to pronounce medical terms and medications!!! I have an account I work in where the work is mostly the same doctor for the whole shift.  Seems lately my hours coincide with Dr. Never-shuts-up!!!  It's not uncommon for him to dictate 10 + minutes for gastroenteritis!  Yesterday he did an 11 + minute note TWICE on the same patient.  What do you all do when all you want to do is SMACK the doctor, lol?  ...

It would not be a bid deal for MModal to TELL these doctors how to dictate..I am sure they are clueless as to what comes over the phone lines. A bunch of cackling hens in the background of an already mumbling dictator is just ridiculous. Why do they not just tell them that phones pick up what is in the background? They now cut our PTO and are threatening us if we don't have all our hours in because THEY are out of work..Oh yea and at least 150 lph and 99.6% is it now accuracy..When I can't understand a doc now, I will not go in past reports, I will leave markers..I will go for a second listen once the report is done but the old Above and Beyond crap is gone!! ...

Have you ever noticed than when a dictator tries to adopt a "conversational" style of dictating, that it is very hard to try and figure out what's being said?  They start and stop several times so that you can't remember what the original line of dictation was, plus they add "you know" every other sentence.  I DON'T KNOW!!!!!  My account is a verbatim one and so I figure out as near as I can what he's saying and then send it through.  ...
