I can't help but think of my days and months of training and if I consistently put out work like I am correcting, I would have been fired in a New York minute!!!  Please don't post the normal you can get out, it's your fault for staying, blah, blah, blah.  I just wish someone would listen as to what a mess this is and the chances of mistakes getting through because there are so many corrections and words that are not even medical or consistent with the report.  Just venting!!

We've been talking to the higher-ups for about 10 years now. I stopped wasting my energy. You can vent on here, but it won't help change a darn thing. You can vent to your supervisor, and she'll just try to calm you down and say things will get better. You can vent in a corporate survey, and they'll just dangle carrots to keep you on board.

Has anyone sent our concerns to the Journal of American Medicine or state board of medicine sm - Oldie

Noting the crazy mistakes we get. My account went to wreck last November. We were told not to change anything unless needed to clarify the meaning. WOW, REALLY? I am not comfortable with Synthroid 113mg. I underline anything that seems incorrect, but I really believe the docs have no clue.

I know my state right now has way bigger problems to deal with - like what is happening with the VA hospitals. NM

attention because the only thing anyone is concerned about is the finished report, not the effort it takes to get it, does not matter that distributors lied and administrators only heard "save on transcription cost."

Doctors? Why should they care? They would all rather die than dictate, as anyone who has done the job for half an hour will realize. They all wish we would all just disappear if they even realize we exist in the first place.

Do you correct the dictator's grammer? My 12 year old niece just recently corrected my mom when my mom said she was laying in bed.  Taylor went on to explain the difference between laying and lying.  I learned that in school but certainly had forgotten that rule. When the dictator says "The patient is laying in bed", do you correct it to lying?  Just curious.  I've had no complaints and I hate sending out dictation with grammatical errors.  ...

Anyone ever had this happen? You spell something and you know it's right, plain and simple - however, the doctor corrects you on it, but its still wrong!! Do you spell it "wrong" -just for him only- to make him happy so he doesnt think you're not doing your job right or do you keep on spelling it "right", which he says is "wrong"?   ...


Correcting VR Question.

Does anybody happen to know if there's a best way to correct VR to hopefully have it actually "learn"?  For example, is it better to completely delete and reinsert the "respiratory" part in an error like "respiratory clear breath sounds" by using my expansion for the capped RESPIRATORY: heading, or would it supposedly learn better if I used case change and added the colon?  In other words, are corrections to existing text better or deletions/insertions instead? I don't ...

Just a small rant here.  WHY in the world do companies tell US, the MT to change this or that when the DOCTOR says it?  An example....VR work, hospital policy is dates in numeric, so doctor dictates June 22, VR types June 22, but we have to change this to 06/22.  Hospital says don't say patient names, but doctor dictates Mr. Smith, VR types Mr. Smith and WE have to change to "the patient".   I mean, wouldn't it be MUCH more simple to just TELL the doctors to say it ...

Correcting Spelling - Which Is More Time Efficient?

Fixing errors on the fly or letting them stay until you spellcheck the entire document?  If it's a they're/there/their type of error, obviously you have to fix it ASAP, but for the ones where your fingers just make a major flub that gets red lined, which do you find to be a better use of time?  I can't stand seeing all those red lines, so I almost always backspace and make the correction right away, but I'm wondering if that isn't more of a time waster than n ...

Just curious - do you all correct any typos AS you type or wait to do it/them at the end of the report?  I do mine as I type; just curious if others have a better way. Also, each day have a different word that won't "TYPE" out right that day - today is  STONE....this is coming out SEONT...dyslexic today.  But each day it's a different word that just won't behave.  Always interesting to see what my fingers will pick each day.  Anyone else have this h ...

There has to be a fine line between vertabatim and what is transcribed. :- Blanks are sometimes left for a reason, and anyone with the knowledge they should have to do QA should know when the value and/or unit of measurement is impossible. The measurement unit the doctor dictated, probably by mistake, was impossible. If that value was so, that duct would have been half the height of the patient. Although, the doctor was ESL, so he may not have known the difference anyway. At least I know ...

Okay, I just started working here and I've been trying to keep my reports perfect.  When a doctor says "past medical history," I've been just typing "medical history" because isn't past and history together being redundant?  I'm still on 100% QA and all my feedback has the "past" put back in.  I'm not on a verbatim account, so shouldn't me correcting this be okay?   ...

we have dictators who compose sentences such as this: "A xx-year-old female accompanied by daughter in secondary to within the last week has run out of her amlodipine, Prozac and clonidine anywhere from 3-5 days ago each of these." I kid you not, that is what this doctor said.  American doc, speaking clear English.  Now tell me, what am I supposed to do with that?  It is a verbatim account, but the last time I let some gobbly gook like that go through I got chewed out for it (!) ...

I worked for Spheris for 4 years and it was great. Mmodal took over almost a year ago and changed everyone to speech wreck, took Outlook email away and put us on some horrible slow email system and now it takes at least 30 minutes of wait time to get the Help Desk on the phone. I am looking for a new position that is straight transcription. If anyone knows of one, PLEASE let me know!!!! Speech wreck has caused by paycheck to be cut in 1/3 at least. Thanks! ...

As per usual, SR came up with a medical worde for a non-medical word and used it throughout a report.  Think about what might sound like "peanuts" and substitute that in sentences like "the patient was allergic to peanuts,"  "She ate some peanuts." ...

I have tried to explain to manager that the program is slow, and does not seem to ever correct a lot of "normal" dictation, so I have come here to ask a question of you.  I feel that we are not getting enough keystroke count for these jobs.  I have had lengthy jobs with small actual counts because of all the repairing (MEing), and pickiing up of nearby conversations, using markers judiciously as requested.  None of these markers are included in the count that I can see, but they a ...

We were supposed to provide 3 job numbers with examples of the same errors being made over and over again. Well, I did just that with numerous jobs because of numerous errors. Not one stinking thing has been done. Does this mean my TSM was just whistling Dixie and blowing smoke and did nothing about the information I provided, or does it mean the ASR technicians were whistling Dixie, blowing smoking and did nothing? I just came across "Benzapril" listed as a medication that Speech Wreckignition picked up.  Granted, that is how this doctor pronounced it, but it should be "benazepril."  (Also notice that the original was capped while the correct is lower case.)  Now, I've seen this a couple times, and from what I understand, the speech system can't just "make stuff up."  Someone has had to have physically type that out in straight transcription (either when it was learning) or c ...

Dictation could not be more clear. Voice Wreck is putting in udder nonsense. Yes, I know it should be utter nonsense. I'm trying to make a point because this is exactly what this sorry excuse of voice recognition is doing. so posting this here. I just can't seem to get any speed on VR and the drafts coming through are really not that bad. I can easily straight type 150-175 LPH, but I struggle with VR so much.  I just got hired at this company and we are only required to get 200 LPH, which is reasonable. Why is it such a struggle for me? I have tried and tried to figure it out. My eye cannot seem to follow with voice and I end up going over and over words. Have tried magnifying the screen to about 14 ...
