Is EMR Mandatory? - survivalmode

Perhaps I should know the answer but I do not.  What is the deal with the move to EMR?  Is it government mandated?  I just picked up a good side job transcribing for a cardiology group in my community and it's projected to last only until they go to EMR..the move isn't happening too quickly...are there penalties if they don't move to EMR?  Who is driving this?

We had EMR at the hospital where I worked in Baton Rouge YEARS before I ever even heard of Barack Obama. I'm not his biggest fan, either, nor am I a fan of his healthcare reform. But let's only blame him for what he has actually done, shall we? Just because you don't like him or his policies doesn't mean you should blame him for things that had nothing to do with him.

The electronic medical record has been around for about 15-20 years. I have not been an AAMT member since the mid 90s, and we had long discussions about the advent of EMR and how it would affect our industry. I don't think many of us around the country ever heard of Obama back then.

Just sayin'. Let's blame him for what he does do and not place "blame" at his feet for things that have nothing to do with him.

Technology and possible cost savings driving it - same as cell phones, video, grocery scanners.

All sectors are feeling the employment problems caused by technology. Fewer bank tellers, grocery store clerks, librarians, etc. The new health care act at least brings more people into the health care system to help stabilize our jobs. Acute care and embracing the technology is the way to go.

I have a few docs I have to see. Now they can check my record to see what tests have been done, my current prescriptions, tests I need, etc. Previously I had duplicative lab tests, prescription confusion, and a biopsy that had not been reported to my primary. I think this will slow the cost of care down and also be safer for patients. When and if I become a snowbird when I retire, I will be able to easily take my medical records with me, if necessary.

This was scary stuff for me and I just kept telling myself that it was not going to happen for awhile...boy was I wrong. Just eliminated positions from supervisor to clerks of several hospitals, clinics. Physician's offices are not even dictating any longer. Administration made it mandatory that the physicians use the system so no need for transcription and also some HIM staff are also gone. For your own benefit, get yourself trained in something else. It did not even take 2 weeks to eliminate the above positions. A very difficult situation to go through.

Why do I keep hearing about the importance of using EMR/EHR systems? As part of the 2009 Stimulus Act, the National Coordinator for Health Information Technology (HIT) was mandated to update regulations to require utilization of an electronic health record for each person in the U.S. by 2014. In other words, under this new HITECH Act, by 2014 it will be mandatory for physicians and hospitals to use EMR/EHR for all patients

I took the course on the HITECH Act which is part of the stimulus plan. Although EMR is NOT mandatory per se, any physician NOT using EMR by their target date in 2015 will get a reduction by 1% in their reimbursement rates from Medicare, and 3% in 2016, and I think it gets even worse from there on.

The government is offering assistance financially to both hospitals and physicians for purchasing the necessary equipment before then. You can Google HITECH act and get more info.

There is a segment of transcription that will not be affected by this. There are individuals and clinics who do not take federal funding. I'm waiting to see how it plays out, but what I hear from those involved in taking only accident cases (Worker's Compensation, slip and fall, motor vehicle accidents, lawyer protected clients) will not be converting. Of course, you can also see if you can get on with a legal office as a transcription in their personal injury division.

EMR/EHR there remains a need for transcription. I have worked for one hospital my entire career and they converted to EMR about 5yrs ago and we still have a very high work flow.

They did implement a point and click system for their physician's offices and plan to do the same for the ER physicians, but its just not feasible for them to do implement point and click with acute care meaning consults, H&Ps, OPs etc. Some reports and hospital encounters require that there be a narrative summary and they just cannot do away with that, so we still remain. We may not be typing many ER reports anymore, but we will have an abundance of other acute care work.

Like I said, our hospital went paperless (converted to EMR - electronic medical record) about 5yrs ago and we still have work.

The EHR is the same way - Electronic Health Record. This does not exclude the need for MTs. As long as there is work that requires a narrative, there will remain a need for the MT.

Point and click templates are mainly used in doctors offices and ER encounters. People keep forgetting about the narrative part.

I have been transcribing documents for an EMR for the last 4+ years. We never get any radiology or ER reports, but we get all kinds of consults, DC summaries, H&Ps, OP notes, and psych dictation, which tend to be pretty long and detailed reports. Our EMR system is very MT friendly. I get a lot of docs that say, "Leave that blank, and I'll paste it in," for various sections of the reports like medications, past medical history, and stuff that they don't feel like repeating but have from before that they can just copy over.

One of the reasons why the money making reports that you mentioned will continue to be required rather than using a point and click or check off system lies in the creed of documented medical records: If it is not in the record, you did not do it and we (Medicare/Medicaid) will not pay you for it. There is concern in the community that the point and click/check off systems can be invitations for increased fraud of federal funds.

You are correct. That is why, even though some facilities have templated/point and click systems, the providers still dictate. Medicare has been known to withhold payment if there are too many errors or the documentation is incomplete.

Drives me nuts the folks who equate EMR with SR/VR. EMR & meaningful use requirements do NOT require SR/VR. Furthermore, EMR and SR/VR are NOT interchangable terms.

I'm sick of the companies & hospitals trying to justify selling/using SR/VR as something they have to do because of the EMR/meaningful use requirements.

Anybody enjoying the mandatory overtime.  Yea funny how we are on mandatory overtime and then get an email that states that if you run out of work, you are required to email them and flex your time and can only make it up after 5 p.m.  So again, you are required to check for your work throughout the day and if there is no work, sorry for your luck and if you want to be "lucky" enough to be employed w/ Nuance, you will then have to sign in and work on your off shift to make up your hour ...

Email received said basically,,,time sheet must be approved, make sure you have the required amount of time or PTO will be added to make up missing and you will need to re-approve it..I dont want to take PTO, I didnt run out of work, THEY DID..I put in my remedy ticket release without pay..How do they get away with that? ...

Just informed that I have to take MANDATORY HIPAA training without pay because I can take it "anytime on my OWN time!"  LOL!!!  The course is 45 minutes long!  It is not enough that we have to learn account specifics "on our own time", read the constant silly e-mails "on our own time", but now we have to do mandatory training "on our own time."  Oh, yeah - and we have to pass the class with a test at the end too!  I feel like I am in a volunteer job!   Hmmmm, I ...

Mandatory Weekends....why?

Why do MTSOs require weekend coverage for every one of their accounts? I have 2 and have work up the yin yang during the week but come Saturday, I'm watching paint dry. My MTSO requires 1 weekend day, at least 4 hours. I changed my weekend shift TWICE to work during the "busy" dictation times and I'm still NJA. Now that it's getting nicer outside, I've got plans and wanna have a life again. The higher ups get off every weekend, why can't we, or at least rotate? I do ...

In my state, they are required by law to pay.  I called the labor board to verify.  By law, they have to pay if you are a non-exempt employee.  I can't file a complaint until the training is completed, but I can do it online the same day as I complete the training.  They (labor board) also advised that I keep emails and attach them to the complaint.  Unfortunately, it could take up to a year to get the money - my state is sloooow in resolving issues. I am curious to know if all this overtime every week is mandatory or not.  MY AM asks for overtime on a daily basis and wants commitments for the week. I have been doing it, but am to the point where I just dont want to anymore, I have things in my life that I need to do or want to do instead of spending MY free time always working, working, working. I was told by someone who attended a recent ICD-10 academy (for trainers) that the OIG is requiring all (current) coders to be certified by the October, 2013, ICD-10 implementation date.  I am aware of the OIG's compliance guidelines, but nowhere in that document do I find anything about mandatory certification. Also, I am not sure if they meant certified in ICD-9 or 10, but it is my understanding that the last ICD-9 CCS/CCS-P exam will be given in March, 2013, and at this point nobod ...

I was thinking... mandatory CMT credentialing might not be a bad thing.  I'd have no problem passing that exam, but would the Indian MTs?  If CMT were a requirement for ANY MT transcribing/editing US-based medical dictation, I think we'd lose our Indian partners pretty quickly. If AHDI could ("would" is probably more appropriate) make this happen, I'd gleefully pay CMT dues and attend all the seminars, etc. ...

I interpret mandatory fields as being those in the upper area that says mandatory plus any fields listed in the CP as mandatory. I interpret the lower fields listed as optional ADT (Alt I) as optional unless stated in the CP as mandatory. There are some accounts having problems with physician names not printing out right and have to be re-researched and entered. These are shown as having a question mark before their name. But, 99% of these have been in the Alt I fields, i.e., optional fields ...
