I started a new client for an MTSO recently. One ESL dictator's H&Ps are so out of order they require a ton of reformatting. This is the order he dictated: History of Present Illness, Past Medical History, Chief Complaint, Plan, PE, Review of Systems, Assessment/Plan. The template has to have formatting changed (bold headings and point size), the guy is very difficult to understand, does not use proper grammar and needs most sentences restructured. He slurs through the Review of Systems so fast there is no way more than 50% of it can be understood. Two plans dictated have to be consolidated into one. He slurs through doctor's names without giving the first requiring research for the right one to CC. My question is: Just how much do the MTSO's expect us to do for that measly .04 cents per line???

They hired you to do that because on your resume you probably said you have experience with ESL's and you're a "seasoned" MT/MLS or something like that. But I don't know the real answer to that. They use you till you can't take it anymore and quit or get fired and then they go on to the next one would be my guess. The next person won't do any better than you could and that is the pitiful part. The company is only as good as the people running it and sometimes you get people that truly want to help and some are very lazy and force you to fill in the gaps.

I am not sure what they expect you to do, but would it be possible to create a template/normal that you could use just for his dictation? I'm on the other side of the fence now and fix formatting errors when I check the work that comes back from the MTSO used by my facility. I fix grammar errors and correct punctuation all day long because the MTs don't want to take the time. I think my skills are worth more than what I get paid too, especially when it comes to fixing other people's mistakes. Maybe you should send it through that way and let QA fix it, or ask your MTSO what they expect from you for .04 cents a line.

For .04 cents per line if all we did was edit, that's minimum wage unless you have excellent dictators.

Question for Veteran: Are these chronic dictation errors ever brought to the dictator's attention? I know they are never addressed with the real perpetrators by the MTSO, but curious to know if the clients address the problems?

The only time we address anything with the providers is when there are technical issues due to headset problems or things that they (or IT) can fix. I work for a private facility and only handle their account and we don't have much ESL. The type of things that I am referring to are the MTs not following the proper format and returning reports that way to be fixed by me. The other errors I see are not dictation errors per se, but incorrect word usage by the MTs and punctuation errors. We have addressed those issue with the MTSO but so far it has done no good. I know everyone works on production these days and I do understand what you are saying. Someone does need to clean up the documents, but who should take responsibility for it is something else. You raise a good point. However, it's frustrating for me to have to correct the same mistakes over and over because the MTs can't (or won't) take the time because they lose money. By the way, the MTSO that we are using is not US-based.

Thanks for the info. I can only imagine what you have to deal with if the transcription isn't done in this country.

It is frustrating in that doctors used to know how to dictate because they had communication with their MTs. Today, they haven't the foggiest idea who is transcribing, what state or even country they live in, and the assume they still have someone right in their town who knows all the geography, doctors names, addresses, etc. What would take them one second to do, such as give the first and last name of an MD they want to CC, can cost us many lines of production to have to track down. The demographic errors also require we work for free to clean up. We are also judged by production so the dictator errors can cost us our jobs. A lot has changed over the last 20 years.

They can no longer find MTs who are willing to do this for the money that is being offered. When they can longer find the MTs with the necessary skills, then they will up the money. My advice (not that it will be taken) is not to do this work for this kind of money unless there is absolutely nothing else you can find to do and you will be destitute and starving on freezing dangerous streets if you don't take the job. The rest of us must must must find something else to do.
For the MT it is either that or starve. and you havea 3 cent a line transcriptionist. You get what y - tynkerbelle

I have never seen pay so low for MTs....I am really pretty shocked to see some of the rates.  I do realize that good ol' 11-12 cpl is never going to be coming back, I know the economy is not good for anyone, businesses included, but I really am wondering how low can I go and still make a decent living?  Now I am talking about somebody with a lot of years of experience in all fields.  I am thinking no lower than 8 for me, and even that makes me cringe, and I am trying to psych ...

The day we suddenly ran out of work in early February was when another MTSO got the account.  For my fellow MTs who are suffering from no work from that account, you can go to TTS, who is still hiring for it. Line rate for employees .01 cent less than TT but they pay headers, which TT did not.  You can make the same (.01 cent more if you decide to be an IC rather than employee).   I hope this info can help some who need the work and no longer have it.  Doubt IMEDX will ever g ...

I'm back to editing after a few years away from it and can't believe how bad the software still is. The company I worked for 3 years ago used better software and the drafts were not that bad. Now I'm with a different company using not the best editing software. Not sure what the docs are using, but the VR really does a bad job, even with very good dictators.
