EPIC question - cloverport

For those in-house transcriptionists and IC at-home transcriptionists who have been through an EPIC launch/dragon dictation, does it totally eliminate the transcription? I am just curious. Wouldn't there be some types of reports that could not be done by either?

Why would some kinds of reports be unsuitable for that?

I don't know about Epic itself, but in most EHRs, in general, all report types can be done by front-end SR, point-and-click templates, boilerplating, importation of information directly from the EHR (labs, medication, history, ROS, VS, etc.) or doctor typing.

My facility uses Epic and Dragon front end SR. We no longer have a transcription department. One person was kept to do QA and the occasional stat. 75% of the work is templated done in SR and the providers do their own editing. Some of the providers still want to dictate and that work is sent to outsource, which is the work that comes back for QA. Our docs were given a choice of what they wanted to do, many facilities do not permit that and make everyone use SR. Really, there is nothing that cannot be auto templated. Epic auto populates a lot of information from a data base and there are a ton of "normal" that can be used. I wish the news was better, but that's how it is. If your facility is converting, chances are your MT department will disappear or the staff will be drastically cut.

One facility I worked for Epic almost completely eliminated the transcription, another there was no change except for now they can transcribe bits of a report instead of a whole report so you end up doing 30-40 reports an hour (sometimes only 1 word a report). Needless to say, you spend a lot of time doing demographics which (at least for the MTSO I work for) you don't get paid.

Eliminated 90% of the transcription jobs at our local hospital. They are a few folks left who type in the hospital, but it was a major blow.

The trend is that it will ultimately eliminate roughly 80% of all our transcription. We are still phasing it in, but the physicians who are basically "doing it wrong" and still dictating one big ol' report are being monitored and will have one-on-one Epic Super User re-training to get them off their pacifiers and the need to dictate in what is quickly becoming "the old way."

The only thing that will remain are the most complex consultations by the "hard" specialties -- oncology, nephrology, and infectious diesease -- and the most multifaceted or troublesome op notes. (Your garden-variety heart caths and hip replacements will be Epic-with-Dragon generated.)

This 80% reduction figure is also in keeping with what the other systems in this area have already experienced.

I'm in a part of the country that is known as a bellwether -- we are early adopters. This is coming, folks.

DQS completely and the hospital will drop MQ. Just curious about that. It seems Epic is the way to go and was not sure if that can be interspersed with DQS or eliminate MQ altogether with DQS. ...

The hospital I work for changed over the Editscript last year. Changeover went relatively smoothly, and we still had plenty of work to keep all the transcriptionists busy (after we stopped outsourcing the overflow we used to have). Recently, the hospital switched to Epic electronic medical record system, and within a week the transcription workload plumeted to probably 40% of our former workload --not enough to keep our transcription staff busy. It seems the doctors/nurses/etc ...

I was wondering why the amount of available jobs has dwindle to almost nothing, MTSO has some excuse like they are now dictating at night more etc etc... did a Google search and low and behond the hospital has put in the EPIC system... Venting session! I am a part-time work-at-home transcriptionist employed at a local hospital as "resource". When I started a couple months ago we had over 1500 jobs in the "cue" with mandatory overtime ... then along came EPIC. The regular employees are working in 2 hour shifts so everyone can get a few reports in ... having under 75 dictations has become the norm and there isn't enough work for them ... and I have had no work for 6 weeks. ...

A HUGE clinic with many different specialties in this area has went to Epic and now have laid off 30 MTs. They said with Epic they didn't need them all. What a shame! Was wondering if any coders here code EMR point-and-click reports and does it make your job harder? I would think that lots of narrative and detail would be more helpful in coding.
