Nuance is certainly the reason for my cut in pay to the toon of around $700.00 a month!  However, how does everyone feel about EPIC further taking over our jobs by 2014??

EPIC is not taking over all of our jobs. Just some of them - EMR is NOT EPIC.

EPIC is only one form of an Electronic Medical Record. It is not the form that is required by law. The only thing the law says is that providers who wish to continue with government reimbursements from Medicare have to have an electronic medical record.

EPIC will drastically cut down the need for MTs, that is true. There is still some dictation being utilized even with EPIC, however, that just simply cannot be done by a point and click system.

I currently work in an EMR system and still do straight transcription.

Woody, I think there is some confusion about Epic. First of all, it is a complete patient management system that includes a billing program, appointment scheduling, an electronic medical record among other things. Some basic information can be templated and some types of reports (routine procedures) lend themselves well to that. Vital signs and some other info can be pulled directly into reports from a data base. However, many facilities have used an electronic health record in some form for years, I've been transcribing using Epic for about a year and my transcription department is busier than ever because the providers prefer to dictate rather than use VR and do their own editing. There are "meaningful use" directives in place that facilities need to follow for Epic. They can use the software as they choose as long as certain guidelines are followed. Some MTs might lose their jobs, but MT is not going away because of EMR.

Some doctors do prefer the point and click method and quit dictating; others still prattle on and on. We are as busy as ever. This is in a (big) hospital specialty clinics setting, no MTSO.

So Epic doesn't get rid of every MT, but a lot of them.

I have watched EPIC and similar platforms - destroy many MT jobs.

The team I supervised for a large university teaching hospital (with numerous satellite clinics) dropped from nearly 100 MT FTE's (full-time equivalents) to about 30 within the span of approximately 1 year as EPIC was rolled out. As each clinic went onto EPIC, I watched the work level dwindle away to nearly nothing and scrambled to reposition people - until it was no longer possible to put them anywhere.

What got my dander up was that none of these people were ever actually terminated...they were just allowed to dangle until they got the hint and left on their own. In other words, the company wanted to make sure they hung around as long as they were needed.

Blecch. What a bunch of slugs we have running the companies in this industry.

I can only speak for myself and my personal experience when I say we are very busy since Epic but it did not start out that way. It is possible that the facilities mentioned here whose employees were "repositioned" or terminated were getting rid of dead weight. Transcription departments generally will have people on staff who don't meet production requirements or just aren't team players. I don't mean to sound harsh but we all know that exists; it also happened in the MT department where I work, but not to that extent. I coulsn't tell if you were speaking of an MTSO. I'm in an on-site situation so perhaps that is different.

EPIC enables a significant amount of "automation" in the medical record, particular as it pertains to repetitive clinical information.

This information was previously dictated - and then obviously transcribed - and is no longer being dictated.

It's not hard to do the math.

We're not to that point yet but I'm pretty frustrated. I'm an MT who always took pride in my work, making sure the formatting was correct, paying careful attention to punctuation and looking things up that I did not understand. Now with Epic, the only thing that seems to matter is how fast a person can type, and some people can really crank out the work. I also know there's a lot of cherry picking going on, which I find extremely frustrating. I'm sorry, this was not a good day; I've worked 10 days without a break because of our backlog and it would not take much to make me cry right now :-(
And we all know doctors and midlevels working off a touch pad or other devices - IAMT

never make mistakes and will not create a pretty scary report in some instances. Found way too many making errors in their dictated reports to us and I hope now they will at least they will be held accountable for their own errors without the blame going to the MT.

The hospital I work for changed over the Editscript last year.  Changeover went relatively smoothly, and we still had plenty of work to keep all the transcriptionists busy (after we stopped outsourcing the overflow we used to have).  Recently, the hospital switched to Epic electronic medical record system, and within a week the transcription workload plumeted to probably 40% of our former workload --not enough to keep our transcription staff busy.  It seems the doctors/nurses/etc ...

Venting session! I am a part-time work-at-home transcriptionist employed at a local hospital as "resource".  When I started a couple months ago we had over 1500 jobs in the "cue" with mandatory overtime ... then along came EPIC.  The regular employees are working in 2 hour shifts so everyone can get a few reports in ... having under 75 dictations has become the norm and there isn't enough work for them ... and I have had no work for 6 weeks.  ...

I was reading a post on one of the other boards about what an EPIC or point-and-click report looks like and it made me think about what it would be like to code a report like that.  Was wondering if any coders here code EMR point-and-click reports and does it make your job harder?  I would think that lots of narrative and detail would be more helpful in coding.  I've never actually seen an EPIC report so I don't know.  Thanks.  ...
