It's probably a little difficult to dictate in perfect sentences, but it just frustrates me so much when a doctor dictates "both" and then names three things. Why say both...both means two! I feel like I should correct it, but I don't want to get in trouble for too much editing.
I don't believe they actually realize consciously that what they're doing is vaguely acceptable, but yay for us, it is.
