Charging on personal accts - Still confused

Okay, I am still a bit confused.  I admit I have been spoiled by being an IC for years.  I have a chance to pick up a couple of side accounts, but after talking to them they just think they can dictate into something at my home and I will magically be able to hear their dictation. I am totally not well versed with all these computer programs.  Can someone explain to me whether it would be worth getting some kind of program to be able to do this?  It seems costly to be able to have the doc dial in and dictate to my home or something like that.  I have looked at Docshuttle, but quite frankly I don't quite get it.

Depends on how many physicians and their preferences. There is always the option of digital recorders, but my clients prefer call-in dictation. This requires a dictation system that uses a special telephony board to capture voice and software, and you would want this on its separate computer (they no longer come bundled). I used to have both VDI and DVI system. There is Winscribe, which is a bit pricey. I have to replace mine about every 1 1/2 to 2 years. I now use a DigiTel dictation system. You can go to digitelsoftware.com for info. You can program in each dictator and prompts individually. Plan on spending about $2,000 to $5,000 for a 4-port system, and then the computer to load it on. Then you will also need phone lines and hunt numbers, so when the physicians call in, they will not get a busy signal. If you are only dealing with a few, it might be cheaper to use a TASP. As a rule of thumb, 2 cpl goes towards dictation costs right off the top. This is a low-end figure with a high-volume workload. Hope that helps.

TASPs will charge per minute usually and per dictator.

yea, that is a bit more than I had in mind. This is a small account and not a lot of dictation weekly. Just about 10 PFT reports a week that he says are too lengthly for him to do on their own system. Don't think I want to invest that much money into something right now in this terrible spot that I am already in. Thanks so much for the answer though.

It's Digitel's "DigiCloud" service, which costs about $90/year plus 33 cents per audio minute (so probably roughly 3 cents per line).

I'm sure there are plenty of competitors, too -- this just happens to be the one I stumbled across as I was doing similar research.

Yes, that would be a better way to go if you have just one dictator dictating longer reports. I use a DigiTel system, and the owner is fantastic, very helpful. Find out what software licenses you will need as well. Good luck and congrats!

Is there a way to ask your IC company you're contracting with to take on these dictators and pay you a bit more to transcribe that dictation you have referred? The MTSO might be willing to even give you a finder's fee.

Since the volume is low, it sounds like spending a lot of money on equipment might not be a good investment right now unless you have other accounts in the works or plan to pursue larger accounts in the near future.

I referred a long time tape account to my MTSO and they were very gracious.

Taking on your own side accounts also involves administrative duties, so that's why I suggested asking the MTSO to step in. This way, you're just typing and being paid to do the work at hopefully a higher rate and not have to deal with the office themselves or keeping track of lines and billing issues, etc. It might make things easier also on getting paid because sometimes small offices put your billing statements in a pile on a desk and you find yourself begging and wondering when they will pay you.

All the physician has to do is pick up the phone, call in and the vf is delivered to you via secure email. You type it and send it back to the physician via dropbox. Voila!

Charging For Faxing

I recently acquired a very small account and the doctor requires me to pick up tapes twice a week, print, deliver the transcripts to his ofice and fax each of the transcribed letters to multiple offices using my home fax machine.  Average letters to be faxed each week is about 30.  I asked for a faxing fee and he said 'No" because the previous MT was not charging for faxing.  What should I do?  I did the math and it seemed I will just be breakeven and not make money cons ...

I think it is a shame that the accounts lost were both to Medquist - the company that screwed the MTs with line counting and pay and screwed its customers by cheating them out of millions of dollars (see Kaiser settlement of $2 million dollars last week).  These accounts are going off shore with lesser skilled labor and quality.   What is ironic is that while MedQuist continues to shrink in revenue growth from quarter to quarter, they will still pick off a few accounts from hospitals ...

I have been an IC for past 13 years with all local physician office accounts. As of July this year, I now have only one left due to the use of the EMR. While those of you are saying that EMR will not phase out transcriptionist..well that may be so, but in my experience it has already phased me out. The doctors have sent absolutely no further dictation since they began using this and they told me they do not plan to. So, I am now out of a job that I really depended on. I have decided I will not accept any MT transcription rates lower than 9 cpl (in an employee position) or 11 cpl (in an IC position).  For medical editing, I'm not going to work for lower than 50% of the transcription rate.  For me, it will just not be worth it to do it for less.  I am willing to go work inhouse and even do other work rather than to accept line rates lower than this.  I see one keeps putting up the same ad with 8 cpl for MT work and 3 cpl for editing a ...
