Accentus Acute Care/ER

Does anyone know why Accentus Acute Care seems to always have the same job ad hiring for acute care MTs that pays a sign-on bonus? It seems as though either they never fill these positions or they are so difficult nobody stays. I would be interested in applying, but the last time I tried to apply with them on the ambulatory side, I could not get the voice file to work. Is this even worth applying for? So, I've been offered a job with both Accentus and Brietner Transcription. I have done my research on both companies, but really struggling to pick one. I really don't want to make the wrong decision. Seems from doing my homework, the potential to make more is at Accentus, but Brietner employees seem pretty happy there too.
