If you make the error again in a different report, then it would be counted again.
I don't know where you'll find documentation on QA scoring as there is no real industry-wide accepted standard.
I speak only from experience having worked in QA for 15 years for various companies. It is a widely accepted practice to count one error for the same mistake repeated within a report. That's all I can tell you. If your QA is trying to ding you for each time, I would go above his/her head.
