Institute an IM system where you can reach out to QA people and when the QA people find a mistake, they could IM you right away before you "forget the report." I worked for a couple of companies in the past that used it, of course, Mmodal has bought them out and screwed it all up. Initially there would be a learning curve, but eventually quality improves when you have some OJT.
