I was the "queen" of not wanting to leave blanks in the old days and would spend sometimes up to 45 minutes trying to research what might have been said, but these days I've adopted a 3 strikes and you are out strategy. If I can't find it after looking for a name 3 times, blank, same with terms, etc. And to cover my sweet little you know what, if they can't pronounce the medicine or the disease or whatever, I don't assume though 99.9% I know what they mean, I just blank away. Garbage in, garbage out has been my philosophy.
