It's easy to hide behind names and epithets. It is the patient's right to have a physician who can be understood. The Constitution of America is written in English, not Spanish, not French, not German. Everybody has a right to clear and understandable communication. Being able to speak English clearly is just as much a necessary a medical skill as is the ability to properly insert a PICC line.
