Put a notation in the comment section that says "Name does not appear on the physician list, nor the facility's online physician list. Unable to verify information with accuracy." Intentional blank. At least if you get called on an error, you can go back and say you completed a responsible search, to no avail. Or, alert your team leader to the situation once you send the job, and ask how to handle it from now on. At the same time, ask THEM to look into providing you with the correct information. Then, if you're called for a blank, refer it to her/him.
