I had a progress note where the dictator stated the patient had colon cancer in the HPI and peritoneal cancer in the Impression. I left a QA marker in both places and left a QA note stating the discrepancy. I got audited on that report, and the auditor filled in the blanks as they were dictated and left the note "blanks resolved." When I pointed it out, I was told "well, I guess the auditor dropped the ball on that one." I guess they "drop the ball" but we fail.
