Stop placing all the blame on the MTs.
You yourself stated the MTs get difficult dictators every once in a while, but you in QA may get several of those every day or so, Obviously you're going to get used to it and easily know what they are saying. Even if an MT gets used to someone difficult, their speech habits may be so lazy that you never know if they're changing something or even hearing what you thought you heard correctly. I've got several of those myself.
Not everyone can afford high end headphones on most MT wages. Some of these people are lucky to earn enough to put food on their tables.
Maybe you should sit down in the MTs chair for a few months and try to meet those impossible line count and quality goals, with constant picking and criticizing of everything you do, and you might find yourself racing through the reports without slowing down the speed on a 2nd listen.
