I make a nice living on 8 cpl, very nice as a matter of fact, working for an MTSO of course. While charging 14-18 would be great, it's still a matter of location. Most suburban and rural doctors won't pay that much. You can charge 10-12 and make a really nice living, especially if you're in Podunk, USA.
