Ambulatory accounts are much easier than hospital accounts. I have only one ESL on four different accounts, so my QC is usually 0 to 1%. I can average 250 LPH when straight typing as long as I effectively use shortcuts. I really slow down though when doing speech wreck. I also never run out of work on ambulatory side. Never have to work weekends either. InstantText software helped me a lot when I do straight typing, but not useful for speech. I have never worked for Nuance on the hospital side, but cannot imagine surviving financially with ESLs.
