I am not 100% focused on patient care anymore. - sm

Posted: Jan 31, 2015

How can I be?  I'm too focused on what I'll get dinged for.

Every other day there are updates to account specifics.  How on earth can one remember every little thing for every single account (I do 5)?

What will the auditor think of this or that?  Should I actually type verbatim even if it makes NO sense?  Should I change what voice wreck typed, i.e., "On physical examination, the patient..." The doctor actually said, "On physical, the patient..."

So, this goes out as my apology to all patients. I'm sorry you are not my #1 primary focus any longer. If you have any questions, call the CEO at Scruance.

Nuance just does not get it that quality transcription depends upon - familiarity with accounts.

...I mean, expecting anyone to be even "decent" transcribing on more than 2 accounts, let alone "perfect"on 5? That, my friends, is corporate insanity.

I basically told my boss I didn't want to do - CINDY

I told her I shouldn't do so and so account anymore, way too many specs to remember and it was my 4th account, so she took me off of it. My head was spinning when I had to do this account.

You Know What, I Don't Care Anymore
May 31, 2011

I started in this profession back in the late 70s, learning "in house, on the job," my first job working from home, was when my kids were approximately 9 and 11 and obviously, not infants.  My mother was dying at that time, and I answered a local ad in the paper to pick up tapes in 1997 and transcribe for 5 cents a line.  Fast forward to the early 90s, we now have the "Lady Madonna Syndrome" -- meaning, I want to stay at home with my six or more children, home school and be SUPER mamma ...

Don't Care Anymore
Apr 09, 2015

If your quality metrics expectations weren't set so humanly-impossible-to-achieve high and you weren't so nit-picky about simple, insignificant errors then maybe we wouldn't have so many accounts on the "consecutive audit failure" list.  And what else do you expect when you throw people into accounts they know nothing about and still expect them to perform at 100%, 100% of the time, while still making lph and keep within the limits of your stupid inactive time and all for pen ...

I Just Cannot Make My Self Care Anymore
Apr 09, 2014

I just can't make myself want to do this job anymore.  Every dictator annoys me so badly.  I can never make my daily line count because I simply can't stand to listen to these people anymore.  The burping and slurping and coughing and sneezing and sniffling and chewing and laughing and mumbling and yelling.... I just can't stand it anymore! I've tried to motivate myself to just do the work, get the lines, and log off, but I find that I more often than not purp ...

Patient Care Specialist?
Aug 29, 2013

The job posting on the job seeker's board sounds fishy to me.  It says "no phone calls, please" but there's no phone number or company name even listed!  I bet it's one of those scammers that want us to give THEM money, and then they'll let us work for them.  ...

Quality Patient Care
Dec 05, 2014

The whole purpose of this new compensation plan is to focus on quality patient care, supposedly.  After 20+ years in this field, including multiple years within an office and seeing how these notes are used, I think I know a thing or two about patient care and how important the quality of the medical record is. Here are some things I know: Doing accounts consistently help you learn what doctors say from note to note.  Likewise, it also means you're more likely to hear a mistake w ...

If Everyone Is So Worried About Endangering Patient Care....
Sep 23, 2010

or a patient dying because of mistakes made in dictation/transcription, then why in the world is it acceptable for any dictator, be it MD, PA, NP, etc. to get on the phone to dictate half asleep, eating, or even probably drunk and get so confused themselves that there is no way you can understand what they are trying to say.  When this happens REPEATEDLY, this person is called a "difficult dictator."  However, if you make a mistake on their transcript or send it to QA, you are the ...

Flunking Audits --- And Nothing To Do With Patient Care!!!!!
Mar 04, 2014

I spend more time trying to figure out what and whether or not I will be dinged for missing or "adding" 'the', 'a', etc. Getting dinged now for leaving output just as it was dictated. Got dinged for missing 'of' even though there was a note that it was dictated really fast. Got dinged for putting the beginning 2 words of one sentence at the end of the preceding sentence, and I remember that report. I could not figure out what the doctor was trying to say! Got d ...

Doctors Using Their Own Templates Jeopardize Patient Care
Dec 11, 2012

I don't know about the rest of you but I have very few male doctors who are careful enough to adequately edit normal templates to meet the patient's particular criteria. It's gets pretty time consuming to do it myself, especially when we are not qualified to do this in every instance. Female doctors almost always attend to these details. NPs, PAs do a much better job than the male docs, but few male docs either care, feel it is their job, or take responsibility for this editing. ...

Excellent Article About The Deterioration Of Notes For Patient Care
Sep 19, 2014

I have always hated it when the doctor crams in every disease the patient has in the first line whether or not the list is relevant to the chief complaint.  I can't think of anything more pointless than having a PA or resident dictate discharges when all he or she does is lift the consultants' impressions and plans and then plug in his or her standard "vital signs remained stable and the patient was discharged in stable condition."  They just chant thru it, as fast as possib ...

"Conduit For Patient Care And Safety"
May 23, 2015

"CAN'T-duit" for patient care and safety is more like it.  The only way we "CAN-duit" is to be able to take the time to produce an accurate and timely documentation of a patient's story and have a compensation plan that is attainable.  Instead we are pressured to produce more for less.  Sorry guys.... not buying it. "Happy" MT Week???   Nope..... not EVEN.  ...

Please Tell Me How To Stay Focused!!! Sm
Dec 04, 2009

I have done this type of work for a long time.  The last couple of years I have a terrible time staying focused on my work and not emailing, texting, surfing the web, etc.  I need the money really badly!  What is wrong with me?  I am not destitute yet - I keep asking myself if that is what has to happen before I can get serious and do my job.  I should be able to do 2000 lines a day but instead I am doing 950 to 1000. I am literally sick over it and worry about i ...

Tips On Staying Focused Anyone?!
Oct 12, 2010

Normally, I'm very disciplined about work.  I have a very stable job, very good pay, great accounts.  I am just having trouble staying on task.  Mind you, I do not have to just push and push.  I just need to relax and complete a report, upload it, do the next one.  I can breaks when I want to.  There is NOTHING wrong with my situation except as soon as I start a report my mind goes to a zillion other things I would rather be doing.  I can hardly get thro ...

Failed Another 3 Focused Audits.
Dec 31, 2013

Yep, that's my 4th within a week. Yep, just as I noted a couple weeks ago, 2 "punctuation" errors and you fail. One spelling error that I have no idea how that could make it through spell check. And a full point for not cc'ing a dictating physician who said to send him a copy - because the CP did not say not to??????? I thought it was policy not to cc the dictator even if they requested such. I cannot even remember any client profile stating not to send a cc to the dictating physi ...

Any Advice On How To Stay Focused?
Aug 25, 2014

Staying Focused/Motivated At Home
Nov 28, 2010

Hi, I was wondering for those of you who work at home, what type of methods do you you use to stay focused at home, ie. environment, lighting, and so forth?  I like working at home but there are days when I'm easily distracted by the littlest thing and it slows down my line count or I need a break for my eyes to get away from looking at words on the computer screen. ...

Tips And Strategies To Help Stay Focused And Productive...
Apr 05, 2011

Hi all, I have to do a Powerpoint presentation this week in front of a group of MT students.  I would really appreciate it if you would be willing to share any tips or ideas that you may use that has helped you remain focused and productive while working in a home environment.  I've been an MT for over 20 years and worked from home for 14.  Other than the obvious strategies that I use like getting dressed, eating breakfast, turning the phnoe off, and dedicating workspace, I& ...

Another Sad Option For Medical Care Is Charity Care. Work With The People
Nov 06, 2014

...bills. Most Saint hospitals and others actually operate as non-profits, and for what they are screwing us over in taxes unpaid, they can afford to help out folks with their bills and still give their CEOs generous stock options and obscene salaries! But, you have to ask for this after your visit, treatment, x-ray, or ER visit. ASK for help if you need it. It is not our fault that suits are in charge of medical care in this country.  ...

I Can't Take It Anymore!!
Jan 18, 2010

I have had 6 (SIX!) reports come through, all having the same mush-mouthed, fast talking, whispering, ESL doctor.  Oh, and the sing-song thing is really about to make me blow my stack. I swear, he is so bad that I can't even understand the patient's name!!!! I am going to pull my hair out.  Yep, I feel it coming.  It takes me forever just to get through ONE of his reports.  He gets so many blanks that I honestly feel sorry for the QA person that end ...

Not An MT Anymore!!
Apr 25, 2011

Working at .075 cpl for ScribeRight I made a whopping 12K in 2009. Wow - let's all run out and take MT classes to jump on that bandwagon.  Anyway, I quit my MTSO pimp and went to work for a well-known Italian restaurant chain in 2010. I work in the kitchen 6 hours a day for 5 days a week (Mon-Fri lunch shift with no nighttime or weekend shifts) and make about 15% more than I did as an MT. I am not an IC anymore but rather an employee, so I saved a bunch on my social ...

I Just Don't Know Anymore
Feb 27, 2012

My bubble has been burst.  I was with eTransPlus when the Q swallowed them up and then I left.  Now, I'm with another MTSO and it's awful.  I've only been MTing for 5 years.  I was an office nurse (LPN) for 13 years before that and I wanted a change.  My first job was with a great company and then the huge acct I was on was bought out by an MTSO, so I had the choice of being laid off or going to IC status.  I chose to be laid off thinking I could draw ...

I Can't Do This Anymore
Aug 01, 2013

I just cannot do this anymore with MMmodal.  I cannot sit here for 14 hours a day for 800 lines.  I am completely broke, have no food in the house, and am more depressed than I have ever been in my entire life.  I'm in a constant state of panic, have been sending out applications and resumes, but am so far in the hole financially I will never get out.  I just don't understand how a company can do this to people and live with what they are doing.  I cannot even ...
