A long rant - Read and weep

Hi, All. I'm out looking for a job (another displaced MT) and have been trying to organize my thoughts when a potential employer asks why my resume looks so ridiculous. Just sharing... I have been employed in the medical transcription (MT) field for over 20 years, both as a transcriptionist and at various levels of management. I obtained the CMT (Certified Medical Transcriptionist) credential in 1990; however, this certification is not, and has never been, acknowledged by medical professionals and is rarely (or inadequately) compensated. In addition, the cost of maintaining this credential by obtaining costly continuing education credits has become prohibitive and disproportionate to its supposed advantages. The organization (AAMT, now AHDI) that awards the CMT has now diluted its resources and stated purpose to the point that its effectiveness and relevance are debatable. The evolution of transcription has been interesting to observe. In the late 1980s, MTSOs (Medical Transcription Service Organizations) came into being, offering hospitals the option of outsourcing the processing of medical records. This offered significant financial incentives to healthcare facilities, including elimination of costly benefits provided to employees, space that could now be utilized for other purposes, and timely 7x 24 turnaround transcription of medical documents. In the 1990s, computer technology advanced, permitting many MTs to work from their homes, which was advantageous for all parties and guaranteed almost instantaneous production of transcribed records for healthcare providers. The development of internet technology opened the door for new possibilities, including retrieval of dictation offshore and the development of MTSOs in third-world countries at an enormous discount. The plight of the transcriptionist has slowly become evident. Working from home has its advantages, but employees rarely meet each other or their employers. All communication takes place electronically, and employee isolation has become rampant. Transcriptionists have largely been unable to network with one another in any meaningful way, and MTSO policies and pay are subject to change without notice and certainly without input from those who are affected. Compensation has begun to plummet without recourse. Companies have begun to merge at unprecedented rates, limiting our options. Speech-recognition technology has begun to replace us, or at least re-define our responsibilities, in a rapidly evolving healthcare environment that has neither the expectation of, nor requirement for, accuracy. With compensation hovering at minimum wage, many MTs have changed employment frequently, though that would not have been our preference. Overhiring by national transcription services has become the norm, and employee turnover has reached unheard-of levels. Despite fluctuations in work availability, employers are unwilling to sustain even the meager earnings of its MTs and subtly threaten that employees' claims to unemployment compensation will be fought to the bitter end. Employer ethics, particularly in an electronic environment, are almost nonexistent. Employer references that reflect employee loyalty, reliability, availability, and accuracy are a rarity any longer, and the department of human resources has become a useless, if not humerous, smokescreen. I am hopeful that my stability in this community can be documented by my personal contacts rather than by my employment references, although I have demonstrated consistently high earnings and reliability even in an increasingly unstable profession.

I know you were just organizing your thoughts. No employer is going to want to hear all of that though, and any employer who does will just check the 'whiner' box for you and move on to the next applicant.

Just keep it simple. I've been working for ___ many years as a medical transcriptionist. I worked as an Independent Contractor. Now I want to do something else.

While it's well written, it's too long and they'd throw you in the "do not hire" pile. But also the problem with working from home is not limited to MTs nor is the problem if pay being cut without input from employees. Companies are now at the mercy of stockholders and small companies, in all sorts of industries, are started with the purpose of being gobbled up by a larger company. This is rampant in the IT industry as well as others, and that's what the owners want before moving onto the next venture.

The days of working 20 years at one place are long over. There is no indicators that will return and the new world economy is going to be you providing your benefits, maybe being contracted by piece work or hour work, and no commitments. I don't predict that will be within the next 1-2 years, but I think it will be the norm for Westerners within the next 15 (and there are several studies and articles you can google to support it).

I am sad because I have been in this industry as long as you and from what I'm seeing, the medical reports will suffer and maybe never be accurate again. If someone is held accountable, it is always the lowest man on the totem pole and that's us.

Really some great insights here. Thank you for posting! Of course it's too long...I was just doing a flow-of-consciousness thing. It's hard to wrap my head around a "global economy" mindset, but what choice do you have?

Sometimes I feel like I have the very BEST horse and buggy outfit in the neighborhood. My horses are perfectly groomed and well fed, and my carriage wheels are quiet and well oiled. Unfortunately, somebody invented the automobile!

I love how you've "organized" your thoughts on our industry. I too am in your shoes. I've recently started job hunting again (after being told to take ANOTHER pay cut) but am finding that HR/recruiters take one look at my resume and throw it in the bin. I've actually had a really good conversation with one recruiter where she took the time to explain to me how HR looks at my resume: Too fragmented (company mergers/buy-outs,etc) and specialized. And it's true. If I wanted another job as a MT, I'd have no problem (without decent pay, of course) but considering the fact that I want to get out of transcription work altogether, recruiters have a hard time seeing any other skills applicable to their openings, especially if I'm trying to get out of the medical field completely, which I am.

It's hard getting employers to see my administrative skill set and ignore the fact that the last 15 years have been with headphones on. Prior to MTing, all I did was administrative work, sometimes at high levels. But because it was so long ago, they don't even look that far down the resume before moving on to the next applicant.

Right now, I feel like my only options are to continue doing this work, which I now hate; spend money I don't have on going back to school; or take a job making more than what I'm currently making but still less than what I really need.

My SIL is studying towards her RN, currently working as a CNA, and she can't believe how little I get paid. She makes more than I do at this point. Ridiculous.

all the job applications are on the computer now, and many are very time-consuming to complete, and I have sent HUNDREDS over the last 3 years. mostly just get the standard "no thank you" e-mail. I have had ONE live interview. How do you explain to someone that you used to make $60K and now you make $25K, working more hours and having more experience? I'm tired. I want out so bad and you know, really, I could make $25K at a fast food place, plus catch a free meal! my back and shoulders and neck and knees all hurt all the time, I don't sleep, and all I worry about is how much my check will be, will it cover the rent. how many more months until I'm homeless. The only resume suggestion I can give you is one somebody gave me. Rather than list out all of your employers and the dates, duties, etc., just put that you were a medical transcriptionist since 19XX, and these are some of the companies/facilities you have worked with. At least it shortens that part and you can squeeze in some jobs you had before this industry killed us all.

I am now working with a program that helps you for free work on your resume and find a good paying job.

I think a lot of these trade schools out there now are a scam, (especially the schools selling MT training). You would be surprised how much our talents could be useful in other arenas in the work force. On my one day off during the week I am at this office putting together a workable resume and getting tips on a whole bunch of stuff from them and from their website.

Good luck in whatever you do. Frankly, MTSO companies do not deserve us and our skills.

Displaced worker programs - Can you recommend one

I haven't heard of programs like this. Are they sponsored by the federal government? This might be really useful.

I think my resume is okay, but I may not be being objective. I would be willing to have it critiqued. Who knows? It might make a difference!

I am using this agency called Suncoast Workforce. They have three locations in my area "southwest FL" and they also use a mobile unit that attaches itself to the local library. I can go anytime. They have computers set up, usually two advisors on hand. It is great. Good luck to you.

Yes, I can understand that my resume makes me look a little unstable. I recently grouped all my MT jobs into one brief paragraph, something like, "Employed as an acute-care medical transcriptionist for various national services, a list of which will be provided on request. The most recent of these is XXX, where I've been employed since 2007."

Like you, I could get another MT job without too much sweat...the question is whether I WANT to work at current wages. After 20 years, almost all my body parts are worn out. Funny how that happens while you're just sitting in a chair. I'm ready for the next chapter of my life, thank you!

I work for a small MT company that put me on a small account with 4 doctors, and I was told that I'd be the only MT.  Within a week it was obvious someone else was taking all the work, and I was told that this other MT was only supposed to take jobs "when necessary."  However, she takes all the jobs the second they show up.  Despite watching like a hawk for 10 hours a day, I get maybe 35% of the dictations overall, and the whole time I know that this other MT has OTHER work s ...

Let me preface by saying that I am not a veteran in this field. I have only been a MT for about a year and a half. I graduated from one of the AHDI accepted schools. I am so tired of listening to their (literally) 14-minute discussions with other residents while they are dictating a 4-minute op note. Mandatory OT????? I am doing jobs that were dictated less than 1 hour ago.  I am getting the worst of the worst ESL dictators.  I don't mind doing some of them but I don't want ALL of them.  Pass the garbage to some others.  My head is pounding try to figure out what they are attempting to say.  Will be lucky if I come anywhere close to 1200 lines today. I used to love transcription. I felt competent and like I was doing a service to patients by making their medical records into an understandible format. Have you ever noticed than when a dictator tries to adopt a "conversational" style of dictating, that it is very hard to try and figure out what's being said?  They start and stop several times so that you can't remember what the original line of dictation was, plus they add "you know" every other sentence.  I DON'T KNOW!!!!!  My account is a verbatim one and so I figure out as near as I can what he's saying and then send it through.  ...

My Big Rant - $48 Versus $81!!!!

For the hours I worked yesterday, I earned a total of $48!!! The breakdown was 60% ASR and 40% typing,and out of 3 jobs I get paid for typing, 2 of them were ASR jobs that I had to F11!!!! If I had typed all of the jobs, I would have earned $81. Talk about a slap in the face, both professionally and reality-wise. Reality with the Q is worse than I thought. So, I got chewed out by QA because a doctor dictated a lab value INCORRECTLY!  It was a genetic lab value, not your normal CMP, CBC or anything like that.  He DICTATED the value and I TYPED the value.  The dictator stated that this was a normal value.  He was quite clear in his dictation and I typed what I heard.   QA got all over me and said that lab value was incorrect, and not normal, so regardless if he dictated it, I should have flagged it.  I told her it wasn ...

So my company is moving everyone from IC to employee on Jan 1. Pros: PTO days, insurance for FT, and they deal with taxes. Cons: I don't need their insurance. I want the "employer paid" portion in my check, which we won't get. Expected daily production went up. Level to reach incentive pay went up and not calculated daily. CPL rate went down 15%! And that's for standard transcription, not VR. Just in production pay alone, I'm losing $100-150 per pay period. Even wh ...
