I have the BOS2 which says the hyphenation should be "a 3-cm lesion," but "the lesion measures 3 cm." I was recently told that the BOS3 changes this and now they don't want hyphens in there at all.
I don't have the money to buy the BOS3 yet, but if there is anybody who does have it, would you be able to tell me what it says about hyphenating or not hyphenating this? Have they changed this now? ...
I hear it occasionally, but on one dictator it's really bad. It's hard enough to hear him. He talks at the speed of light, runs his words together, moves away from the mouthpiece and his voice fades into oblivion, then that blasted snapping noise on top of it all. Can someone explain to me (or point me in the right direction of where to look) how do I know when to use IV versus 4. I'm doing searches but confused as both are coming up.
Example - grade II (or 2) patellofemoral chondrosis, grade 4 (or IV) osteochondral defect. My report is not looking uniform and unsure which to use anymore or where to look to make sure I'm correct.

All of my searching brings up only FIGO staging, never grading. I even checked the FIGO web site. So, when a doctor dictates it as grade, should it be automatically changed to stage? TIA ...
