The auditor should determine what percentage of the dictation the physician got correct and then the MT should have at least 99.6% of that correct. I had a doc today say, "There was no palpation to palpation over the remaining joints." I left it, didn't even blank it, mainly because I could clearly hear it and knew they had mis-spoke but how am I supposed to know if they meant pain or tenderness or what? Most of my audio today was extremely lousy and I had left enough blanks as it was. What's with these people dictating where it sounds as if they have the phone on speaker yet they're in another room down the hall? omg
