I am sick of getting dinged for "illogical" text. - sick of this

Being that I have 30 years experience, have been here for about 8 years, and have never had bad audits but am now being failed and under performance management, I figure I am on their target list, and it is just a matter of time before I am out of here either by their hand or my hand (haven't landed the new job in the new career yet). So, I might as well go down with a good fight. I have been dinged for "illogical" text despite leaving the text as dictated. I had 1 that I am sure I would get dinged for, but I had no idea how to fix that since I had no idea what exactly the doctor was trying to communicate. So I sent the job number and statement that there was illogical text in there because I did not know what the doctor wanted and thus could not fix it. LOL I might as well have fun with their arrows and shoot a few back.

That's EXACTLY how I would do it. Then, if you get audited and there are points taken off for the blanks, challenge it! If an auditor is able to fill in blanks, it is supposed to be Educational Feedback only, with no points counted against you. That is what my QC, PC, or whatever they are called now, told me.

[ In Reply To ..]They are also used for unclear meaning. Leave the blanks where the context of the sentence gets sketchy. Leave several blanks per sentence and/or paragraph if that's what it takes. As I stated above, you should not be counted off on an audit for blanks, so use blanks where context is unclear. For example...
If this was dictated, "The patient is a rabbit every other Sunday and has an ejection fraction of 65%."
I would edit the report to show, "The patient is a ______ and has an ejection fraction of 65%."
OK but sometimes it is the WHOLE thing that makes no sense. - Not one or two particular words.

Perhaps if there is an auditor reading this post, they can chime in and help with this. However, I think I can safely assume that leaving a blank for an entire paragraph would be unacceptable.
I think the best thing to do is leave several blanks throughout the sentence and/or paragraph. Even if you do something like this, "_____ the _____ an ______ the patient _____ with ____ the _____ the _____ the ..." you should not have points taken off for it.
Do you have a good QC/PC person who will answer your emails? This would be a fantastic question to ask her! I'm sure she's familiar with this specific dictator(s) and could give you suggestions on how to handle illogical text.
I mean when I understand every single word - but the sentence(s) do not make sense.

This was a new account, and she asked "is this account really that hard, that you have to leave this many blanks?" Then we got e-mails that we were leaving too many blanks, and to spend more time figuring them out, using previous reports, repository, and research, along with e-mails saying we were spending too much time doing research.

[ In Reply To ..]The MTSOs want us to handle dictators who do not form words and just utter monotone animal noises through a ROS or PE by filling in what they normally dictate in places were no MT, QA person or doctor would be able to understand it. It's amazing how far they will go to blame us for chronic doctor errors.
As for jibberish, this is something I rarely experienced until doctor's introduction to EMS. When they are dictating while trying to figure out how to use point & click and find prior records, they say things that make absolutely no sense.

I've had an entire paragraph of jibberish dictated by a drunk-sounding Asian with cheeks full of marbles where I left blank after blank with the few words I could pick up. Then, sent it to QA. If I have to listen to that crap, someone else is going to too.

I will let a long report go with two blanks without sending to QA; if it is a short report or more than 2 blanks with clear dictation, not a bad audio file, I will send the report on to QA which 9/10 times they too can't figure out what was being said.

MM ASR stinks royally!!!!! This neurology consultation is so full of made-up output, I just might F11 it (oh goody, I will then get to hear from the QAM). We have an account on 100% QA which is checked by MMEs. No 2 grade alike. If the doctor doesn't dictate "Laboratory Data" so you don't put it, you get a nasty note, "'Laboratory Data' should have been inserted here." I have a very low pending rate. I very rarely have to pend, but there was a discrepancy in the header so I pended it and explained that in the box. Here I am, getting shuffled around in the 'cesspool', a dozen unfamilar accounts. You have your decent accounts with just the basics. Then, you have the CPs that are a long as novels and, if you miss something, you will hear about it, the fact it is in the CP and you shouldn't make such a mistake. I already spend so much time reading CPs, that I then get warnings that my production has fallen. So much for a consistent layout of CPs. I have to hunt for desired PE format, etc... ...
