i agree, it was wrong. We are suppose to be aware of every period, comma and i and e. Where I used to work before, they called us "medical/English language specialists". That's the title I enjoyed the most!

I just got dinged on an audit too, for stupid stuff. It frustrated me to no end that out of all the reports I did that week they found the one report where I messed up. I owned up to my mistake and learned from it and you can too.
Now it seems the only time we hear from MQ is when we have screwed up. They have us trained to be afraid to answer the phone or check our email because the only news is bad news. They only let us know the reports that "fail" their little audits with less than 98%, but never share the ones that pass. I wonder why that is? God forbid they give some positive feedback or actually tell us we are doing a good job. Something about catching more flies with honey than vinegar, but maybe in India they like vinegar.
I know they are doing more spot audits and quality checks on certain accounts and dictators because of all the poor quality slipping through, just something to keep in mind. Hang in there. This job does not define who you are, it is a paycheck and that is all.

There have been quite a few complaints lately about getting audits back where the auditors have inserted or taken away words such as: the, of, a/an, etc. The omission or insertion of these words when not dictated is something that should be captured as an error. However, the point value for such an error usually is zero. This type of error is usually a feedback and educational opportunity error. We have an account on 100% QA which is checked by MMEs. No 2 grade alike. If the doctor doesn't dictate "Laboratory Data" so you don't put it, you get a nasty note, "'Laboratory Data' should have been inserted here." So the next report, you put in "Laboratory Data" and get a nasty note "He DID NOT dictate this header!" Can't win with this dirtbag scum-sucking cluster*&^% of a company. ...

Okay, I just started working here and I've been trying to keep my reports perfect. When a doctor says "past medical history," I've been just typing "medical history" because isn't past and history together being redundant? I'm still on 100% QA and all my feedback has the "past" put back in. I'm not on a verbatim account, so shouldn't me correcting this be okay? ...
I have a very low pending rate. I very rarely have to pend, but there was a discrepancy in the header so I pended it and explained that in the box. I get a QC1 review (I know this doesn't count but it doesn't make it any less stupid!!) with a 1.0 major error for the very thing I pended it for. This in no way makes sense. Okay, I have had the suspicion that MQ programmed something when you bring up old reports when typing a discharge summary that the H&P doesn't show up. Today, the dictator doing the DS plainly stated that she was reading right off the H&P. I went to check in the old reports to maybe get some quick lines, but it wasn't there...as usual. When you suspect that a word may have been omitted from dictation, do you make a note for this to be reviewed or do you think that is overstepping your bounds and basically saying the doctor made a mistake? For instance, this ESL doc said "Bacterial endocarditis is not particularly recommended, but the patient has had Augmentin or amoxicillin and may be given a single dose.." I suspect he forgot to say bacterial endocarditis "prophylaxis" is not particularly recommended...etc. ...

I am a remote transcriptionist for a large hospital. We recently went live with VR Nuance/escript. Ever since go live I have had the continuing problem of typing a voice file and will lose a keystroke, have to backspace, sometimes hit the same key 3-5 times before it will show up on the screen, sometimes taking me an hour to 2 finger type a report. MQ uses Instant Text for their line counts. I knew i had it for word expanders but prefer to use my own WEs. Well, they have a line counter too but I haven't had the time to check it out.
So, today, I took about 5 documents, copied and pasted them into Word. MQ requires 65 CPL, Word requires 73, so I should have come out behind in Word each time. Instead, I came out behind in every document, the least being amount 11 lines and the most was 29. T ...
