Hi everyone. I'm kinda new on this board. I have a question. I'm wondering how many docs you generally get working for online companies. Do you get new docs all the time? Or do you generally work for the same docs on a regular basis? I work for a hospital, and I've been asking MTs where I work, and it's coming up that most of them seem to get the same docs all the time. I've only been there a year and a half, and up until a couple months ago, I was getting a lot of the same docs also. However, in the past couple months, I've been getting on average about 80 percent different docs every day, and it's getting quite frustrating. (To give you an idea, I've gotten nearly 60 different docs in the past 4 days, whereas it used to be around 20 or so.) My line count is down and getting worse all the time because of the frustration; I can never get used to the same docs because I'm always getting different ones; and I have to continuously look up sample reports, all day long. We have people where I work who assign work (it's not automatically generated) and, as I mentioned, everyone I've talked to so far says she gets pretty much the same docs all the time. I just wondered what happens with regards to this issue in online companies. I've actually been putting out resumes for a different job because it's getting so incredibly frustrating, and I'm thinking if it's like this everywhere, I should probably stop doing MT altogether (although, like I said, it wasn't like this until just a couple months ago). Thanks to everyone who responds courteously.

It depends on what kind of facility your online company assigns you to. If you are assigned to a hospital, you will get a lot of doctors, which makes it more difficult to get the same doctors frequently enough to get used to them. This is one reason why I hate working for hospitals. If your online company assigns you to a small facility, like a doctors' office, there will be fewer doctors and it will be easier to get used to them.

I agree with everything you said about continuously looking up sample reports and being frustrated and your line count going down.

My current online company assigned me to a small doctors' office with only 8 doctors, so it is easier to get used to them.

Thank you. Yes, it is helpful. I counted this week, and I received almost 70 different doctors in 5 days - every day I get about 17 or 18 doctors, and almost all are different from yesterday or the day before, etc. etc. It didn't used to be like this on my job, and I'm talking w/other MTs there who say they get the same docs all the time. It just seems to me that it's much more productive to try to assign the same 20 or 30 docs to each MT. We have about 10,000 dictators, so I understand if we don't get the exact same docs all the time (though some MTs here do), but when you get different ones all day today, and you're just going to get different ones all day tomorrow, too, it gets to be frustrating. I work for MModal and I type 16+ hospitals each day, and rarely get the same dictator in a week. If I could just get 1 hospital per day I would be forever grateful!.

The company I work for is giving me a hard time and only letting me type for 1 physician in the clinic I had worked for since 1997 before going electronic. Oh, and now he is on vacation and they are not putting me on any other accounts since they are all very current. I am trying to get another job too since right now my company is only letting me get 600 lines when I do type.

I didn't say I get 1 hospital per day. I work for a conglomerate of hospitals. We have dozens and dozens of differents sites, and we have at least 10,000 doctors.

I do different kinds of reports all the time; I don't just do 1 or 2 or 3 kinds of reports. I do all specialties and all different kinds of reports.

Decide to work for a company remember that you will be getting paid per line. There are two nationals in this industry, MModal and Nuance. The rest are mid-size to small companies.

The line rates vary between 4 to 6 cents per line for speech recognition and 8 to 10 cents per line for straight typing.

If you land with a company that is willing to pay you 5 to 6 cents for speech recognition and 9 to 10 cents for straight transcription, with busy accounts that dont run out of work you can do good. But even then there is no guarantee that you will be doing a certain amount of physicians.

The clinic (physician's office) that above poster speaks of is the exception to the rule, but the problem with that is job security as more and more clinics are moving to point and click systems and people are losing their clinic jobs/physician's office jobs without notice.

Sounds like you work for a health network that has their own pool of transcriptionist, which is good because they dont sound too likely to be outsourcing to a company anytime soon. Maybe try doing weekends or part-time for a compnay first to get the feel of it, but I wouldn't just jump right into it if I were you.

I am assuming you are paid hourly with good benefits and what sounds like guaranteed work. Do you really want to give that up for a job you are not sure about?

Not to discourage you in anyway just saying to make sure you think everything through and weigh all the pros and cons before making any decisions.

By the way, I happen to work for MModal and I actually really like it. My hospital was outsourced to them and I have good line rates and came in the door as a tier 3. I am fortunate in that I also work on very busy accounts and yes, I do multiple hospitals and physicians as well.

I was trying to stick it out with my current small company since I work as a morning stocker for a hardware store from 5 a.m. to 9 a.m., but I think a big company is the way to go if I want to stay at home and do the transcription work full time again. I only have 2 years of hospital work experience from when I worked at a hospital a long time ago. I just hope I can get a company to take me at least part time at first.

thanks for the info. i have worked in the past by production - for about 5 years - so i know what it means to have to get paid only what you're worth. i'm not afraid of that. the company i work for has "threatened" (as many of the MTs i work with call it) to go pay-for-production many times, which is why i wanted this job, but they still haven't done it. (i find too many people these days are afraid to get paid what they're worth.) i'm really just looking right now to find out what online companies do about same/different docs. when you say, "multiple hospitals and physicians," what do you mean? do you have 20 or 30 docs, or do you have 50 or 60 docs, etc.? thanks!

went to MDI, which was bought by Transcend, which was bought by Nuance. If you get on with Transcend side of Nuance, you CAN get just one hospital. I am told they have lots of open requisitions.

thanks for the reply. can you tell me what mq, mdi, and the other things you talk about are? sorry if this sounds like a dumb question, but i've only worked in the transcription pool of a hospital conglomerate (we have thousands of different dictators, though, so i have lots of different specialties and report types i've worked on) -- i don't really know anything about the online companies. thanks!

...since this change just happened a couple of months ago. If fellow employees are still getting the same doctors in a similar fashion as you were, but they switched you and not them, I would ask at least why the change two months ago.

Making someone jump around makes no business sense. It cuts down on your productivity, which in turn cuts down on the amount of work an MTSO can bill for. I personally think its laziness on the MTSOs part, although with the platform systems we have now it should be easy to queue specific work.

I've worked for two companies in the past 20+ years and have always been assigned doctors. My last account was an acute care facility and we'd be assigned by department. In my current company, I transcribe for about 5 facilities in different states, but the same doctor in some and the same departments in others. In other words, I still get the same doctors. Even if I help out with other work or backlogs, they still assign me that new doctor/department in a batch, so I will get the same doctor/doctors in a row.

There's a reason that the good companies do this, and the companies that you hear the most complaints about, don't. It's a terrible system to have MTs do random work, and I never heard about it until the offshoring corporate MTSOs entered the scene.

The only reason I can think of, as I said, that a company wouldn't do this is laziness - it's much easier to throw an employee in a pool and let them flounder, than to be on top of things and know how to route the work most efficiently.

thank you! i totally agree with you, that it is unproductive to just have MTs do random work, willy-nilly doing reports for whomever. i mean, isn't that the point of having people ASSIGN the work rather than just having systems that randomly bring up the next report in line? honestly, i've thought long and hard about this, and i actually sent an email to my immediate manager last night, but i recalled that email -- i guess i'm cynical, but i tend to think that little is usually done about grievances. i've really just decided to look for something else, which is the reason i posted this message. i do understand that nothing will ever be exactly the way we wish it to be, but i also think that doing reports for 70 different doctors in 5 days is a little much for me. maybe others can do it, but i think it's silly and unproductive. my line count consistently suffers and, though i am paid hourly, i am never able to get the bonus, and i certainly won't get the best raise i can this year if my line count is just above the minimum. thanks.

I am training someone and I am now reminded how far I have come, how much I have learned, and the things I take for granted and assume others know.   Basic things, just proper English, not even taking into consideration the medical terminology.  Knowing whether a sentence makes sense or not.  We really should be paid better.  But it is nice to feel smart sometimes....and you know anyone who can do this job really does have to be pretty darn intelligent.   Fello ...

I work in the path department and when a malignant case comes up that requires a CAP, I defer to the CAP summary.  The docs dictate "lymphovascular . . ." but the CAP summaries will have "lymph-vascular . . ."   The problem is today I was checking the CAP protocol for the Lung CA and the word was used both ways.  In the actual protocol itself, there was the hyphenated version as a header, i.e., but then in the explanatory notes at the end of the protocol  there' ...

Maybe old posting on the internet...not sure, or if all jobs are taken.  Hard time finding the "transcription" postings and then led to a test.  Not that long of a test but then states you did not pass then prompts you to a sign-up for medical terminology classes.  Not sure if this is just a class sign up come on.  If I were going to take a class, it surely would not be in transcription as there are no jobs out there.  We are given other avenues  ...
