In fact, you are not supposed to be looking at your own chart, both are HIPAA violations. You need to provide written authorization to the provider. I’m not sure if the charge is appropriate as this is probably continuity of care. If your new PCP wants the records, let them request what they want. There is nothing wrong with a receptionist taking vital signs. In some practices, doctors room their own patients due to staffing shortages.
