My OM (now gone) sent out a general specs document which we were to follow for all reports. It says to include patient's name as dictated, HOWEVER, I only include it if it is an account where the name can be verified or if the dictator spells it. If I am phonetically entering the name then I use the patient. I also thought the patient should be left out for confidentiality - who knows.
