you said that you were working as a transcriptionist, and was wondering what would happen if the doctor said "right lower extremity" when you knew they meant "right lower quadrant". In theory, if you are working as a transcriptionist and that situation arises you get appropriate clarification and the coder never ses that error. Yes, coders must query when appropriate, but you will find out, when you are working as a coder, that a great deal of this information you are going to have to be able to figure out from reading the chart. You don't want to over query the doctors either. I apologize if you thought I was "off topic".
