If you feel like my answer was condescending and presumptive, it might be a bias on your part. I have respect for both professions, which is what surprised me when I thought the original poster would let that kind of error go through to the chart. That is more of a transcription issue... the coders will be able to get the information they need from the chart because they use other things than just the diagnosis, but as a transcriptionist, I find that kind of error bothersome.
