If the report does not contain what you need, you query the physician to amend the record. Most facilities provide education for providers to prevent problems like this. It isn't a big deal. We do it all the time.
If you are thinking about this while you are transcribing, it is likely that you are thinking about spelling and grammar errors or other transcriptionist errors. Those kinds of things do not usually bother us. The problems that affect us are much larger.
